Akut periferik arteryel oklüzyonlarda cerrahi tedavi ve sonuçları; 58 olgunun literatür eşliğinde retrospektif incelenmesi

SURGICAL TREATMENT AND RESULTS OF ACUTE PERIPHERAL ARTERIAL OCCLUSIONS; RETROSPECTIVE ANALYSIS OF 58 CASES IN LITERATURE

Öz

The cases who were admitted to our hospital emergency service and performed surgical treatment by cardiovascular surgery clinic due to acute peripheral arterial occlusion were analysed. The cases who were admitted to our hospital emergency service with the complaints of function loss, coldness, pain and cyanosis in their extremities and performed embolectomy operation by cardiovascular surgery clinic between january 2012 and january 2015, were analysed retrospectively about gender, age, localisation of the arterial occlusion, etiologies-accompanying diseases, the time from the onset of symptoms to surgery, surgical procedures, postoperative follow-up and results. 58 patients were included in the study. 53,44%, (n=31) of the patients were female, 46,55%, (n=27) of the patients were male, average age was 71,39 ± 14,88. The arterial occlusions were 5,17% in radial artery, 22,41% in brachial artery, 17,24% in iliac artery, 1,72% in aorta, 51,72% in femoral and popliteal artery and 1,72% in the distal part of the trifurcatio. A total of 66 embolectomy operation were performed. 77,58%, (n=45) of the cases admitted in the early period and 22,41%, (n=13) of the cases admitted in the late period. 58,62%, (n=34) of the cases diagnosed with arterial system color doppler ultrasonography. In 41,37%, (n=24) cases history and physical examination were enough for the diagnosis. Cases were also analysed about the etiologies-accompanying diseases and atrial fibrillation (36,20%, n=21) was the most common one. As the result of the surgical procedures, 75,86%, (n=44) of the cases were discharged by healing, 6,89%, (n=4) of the cases were referred for further evaluation and treatment, in 8,62%, (n=5) cases amputation was suggested and 8,62%, (n=5) cases were died. Their average hospitalization time in our clinic was 2,89 ± 4,42 days and during their hospitalization in our clinic no infection has been detected. Acute arterial occlusions are extremity and life- threatening clinical situations when they were not diagnosed and treated in the early period. However, the success of treatment depends mainly on the patient's contact time. Investigation of the cardiac causes and applying prophylaxis to these causes are important for prevention of the recurrent occlusions.
