Comparative analysis of laparoscopic sleeve gastrectomy and laparoscopic Roux-en-Y gastric bypass applied in the surgical treatment of morbid obesity.

In this study, it was aimed to determine the efficacy of laparoscopic sleeve gastrectomy (LSG) and laparoscopic Roux-en-Y Gastric Bypass (RYGB) applied in patients diagnosed with morbid obesity in weight loss and the treatment of comorbid diseases. Between September 2014-September 2015 16 patients having been applied LSG and 16 patients having been applied RYGB were enrolled into the study. Demographic data of the patients, their comorbidities, their weight loss during follow-up and the effect of treatment methods on the treatment of comorbidities were evaluated. 13 of the 16 patients in the LSG and the RYGB group (81.2%) women and 3 (18.8%) were male. While mean age for LSG group was 32.2, in the RYGB group it was determined as 46.6. The average BMI and weight loss for 1, 2, 3 and 6 months post-operation are presented in tables 1 and 2. It was found that the patients in RYGB and LSG groups diagnosed with hypertension (3 patients, 4 patients), and type 2 diabetes mellitus (respectively 5 patients, 10 patients) (using oral antidiabetic drugs and insulin) started not to need or had a lower dependency on drugs on an avarage 2 months after the operation. While there were only 2 out of 5 patients in LSG group diagnosed with type 2 diabetes patients using insulin, all patients in the RYGB group had more than 5 years insulin history. Mortality was not observed in any patient. LSG and RYGB, providing excellent weight loss, are two key acceptable methods of bariatric surgery with low complication risks. In our clinic, while LSG is preferred in particular, for the patients using oral antidiabetic drugs and relatively at younger ages with no limitations of mobility and for those regarded as easy-to- adapt to the post-operative process, on the other hand, for those presenting high doses of insulin requirements and despite this with a bad glycemic control who are at the same time relatively older and have mobility limitations RYGB is particularly preferred.

MORBİD OBEZİTENİN CERRAHİ TEDAVİSİNDE UYGULANAN LAPAROSKOPİK SLEEVE GASTREKTOMİ VE LAPAROSKOPİK ROUX-EN-Y GASTRİK BYPASS’IN KARŞILAŞTIRMALI ANALİZİ.

Bu çalışmada, morbid obezite tanılı hastalarda tedavi amacıyla uygulanan Laparoskopik Sleeve Gastrektomi (LSG) ve Roux-en-Y Gastrik Bypass’ın (RYGB) kilo kaybı ve yandaş hastalıkların tedavisindeki etkinliklerinin irdelenmesi amaçlandı. Eylül 2014-Eylül 2015 tarihleri arasında LSG uygulanan 16 hasta ve RYGB uygulanan 16 hasta çalışmaya dahil edildi. Hastaların demografik verileri, yandaş hastalıkları, takipte kilo kaybı ve tedavi yöntemlerinin yandaş hastalıklar üzerindeki etkinlikleri değerlendirildi. LSG ve RYGB grubundaki 16 hastadan 13’ü (%81.2) kadın, 3’ü (%18.8) ise erkek olup, ortlama yaş LSG grubunda 32.2, RYGB grubunda ise 46.6 olarak belirlendi. Operasyon sonrası 1., 2., 3. ve 6. ay ortalama VKİ ve kilo kayıpları tablo 1 ve 2’de sunulmuştur. LSG ve RYGB gruplarında hipertansiyon (sırasıyla 3 hasta, 4 hasta) ve tip 2 diabetes mellitus tanısı olan (sırasıyla 5 hasta, 10 hasta) (oral antidiabetik ilaç ve insülin kullanan) hastaların operasyon sonrası ortalama 2. aydan itibaren ilaç kullanım gerekliliklerinin ortadan kalktığı veya azaldığı saptandı. LSG grubundaki tip 2 diabet tanılı 5 hastadan sadece 2 hasta insülin kullanmakta iken, RYGB grubundaki hastaların tamamı 5 yıldan uzun süredir insülin kullanmakta idi. Hiçbir hastada mortalite gözlenmedi. LSG ve RYGB bariatrik cerrahinin, mükemmel kilo kaybı sağlayan ve kabul edilebilir oranlarda düşük komplikasyon riskleri olan iki temel yöntemidir. Kliniğimizde özellikle oral antidiyabetik ilaç kullanan, göreceli olarak daha genç yaştaki, mobilite kısıtlılığı olmayan ve operasyon sonrası sürece uyum sağlayacağı düşünülen hastalarda LSG yöntemi tercih edilirken, yüksek doz insülin gerekliliği olan ve buna rağmen glisemik kontrolü iyi olmayan, göreceli olarak daha ileri yaşlardaki ve mobilite kısıtlılığı olan hastalarda özellikle RYGB tekniğini tercih etmekteyiz.
