The cutting seton technique with the tractus medialisation in the long tractus anal fistula (New technical description).

ABSTRACT The technical description and results of the new cutting seton therapy which is converting the long anal fistula tract into low transsifink tract is presented. In this study, a new treatment method and long-term results are described. We applied this treatment on the long tract transsifinkteric fistula whose external openings were above 5 cm from the anal canal. The opening points of long tract transsifinkteric fistulas in the anal canal were localized in 6 male patients with ages of 37- 60 years old. Then the dissection was progressed upto intersphincteric distance around of the external opening and the dissected lateral part of the fistula was excised. A seton was put into remaining medial tract and the lateral end of the seton was passed through the tunnel which was opened subcutanously and then pulled into the anal canal. This techniqueis named as ‘Fistula Tract Medialisation' that is the remaining fistula tract was converted to the low transsifinkteric tract with the seton. The skin incision was partly sutured from the medial portion and left open for drainage and seconder improving. The remaining tract and surrounding tissue were cut within 6-18 weeks by narrowing cutting seton in two weeks apart under local anesthesia. A complication was not observed except a wound infection in a case and temporary minimal anal incontinance in 3 patients. The complete recovery was observed in all cases which had at least 28 months of follow-up period. We express that the defined technique is an effective and safely applicable method in the long tract fistulas despite the limited number of included cases.

UZUN TRAKTLI ANAL FİSTÜLLERDE FİSTÜL TRAKTI MEDİALİZASYONU İLE BİRLİKTE KESİCİ SETON TEKNİĞİ (YENİ TEKNİK TANIMLAMA).
Öz
Uzun traktlı anal fistüllerde fistül traktının aşağı transsifinkterik trakta dönüştürülüp kesici setonla tedavisinin teknik anlatımı ve sonuçları bildirilmiştir. Bu çalışmada anal kanaldan 5 cm.den daha uzakta fistül dış açılımı olan uzun traktüslü transsifinkterik fistüllerde uygulanan yeni bir tedavi metodunun tanıtımı ve uzun dönem sonuçları bildirilmektedir. Yaşları 37-60 arasında 6 erkek hastada saptanan uzun traktlı fistüllerin anal kanal içindeki açılma noktaları belirlendikten sonra dış açılma deliği çevresinden yapılan diseksiyonla intersifinkterik mesafeye kadar ilerlenmiş, fistülün diseke edilen lateral bölümü eksize edilmiştir. Mediyalde kalan rezidü traktüse seton yerleştirilip setonun lateral ucu subkutan olarak açılan bir tünelden geçirilip anal kanal içine çekilmiştir. ‘Fistül Traktı Mediyalizasyonu’ olarak isimlendirilen teknikte rezidü fistül traktı setonla birlikte aşağı transsifinkterik trakt haline dönüştürülmüştür. Cilt kesisi mediyalden kısmen sütüre edilip drenaj ve sekonder iyileşmeye bırakılmıştır. Yerleştirilen seton kesici olup iki hafta aralıklı olarak lokal anestezi altında daraltılmak suretiyle rezidü trakt ve çevre dokular 6-18 hafta içinde kesilmiştir. Bir olguda gelişen yara yeri enfeksiyonu ve 3 hastada gelişen geçici minimal anal inkontinans dışında bir komplikasyon gözlenmemiştir. En kısa takip süresi 28 ay olan vakaların tümünde tam iyileşme gözlenmiştir. Uygulanan vaka sayısının azlığına rağmen tanımlanan tekniğin uzun traktlı fistüllerde güvenle uygulanabilir ve etkili bir metod olduğu görüşünü ifade ediyoruz.
