To Be Diagnosed With A Malignant Disease: A Life Crisis Should The Bad News Be Broken?

Abstract

cancer is a life crisis for many people, Studies have shown that three out of every four families will one day be touched by cancer. That means every one can be involved in at least one close relationship with sorneone who has this malignancy. During this life threatened experience, patient-doctor communication has a cruciai importance, particularly at the moment breaking bad news and this communication also have implications for the well-being of patients and family members. Breaking bad news İs a difficult task for the physcians beacuse of context, the characteristics of patient-doctor communication, the patient's reactions, not to know how to cope with thin kind of situaiions and the others. Clİnical experiences and literatüre have shown that breaking bad news generally were done badly because of fear of blamed, of the unknown of expression of emotions and of not knowing ali the ansvvers as well as personal fear of iliness and death. As mentioned İn the literatüre there are three methods of breaking bad nevre approach: the blunt and unfeeling way, the kind and sad way, and the understanding and positive way. Recommendations has been made that the news should be presented in a thoughtful, caring and sensitive way that shows respect and the empaty for the receivers. Recently communication skills and medica! practice have become an area of intense interest and emphasized that there is at present liitle traîning on the process of delivering bad nevvs.

Öz

Kanser gibi olumcul bir hastalık tanısı almak birçok insan için bir yaşam krizidir Çalışmalar her dört aileden uçunun gunun birinde kanser olgusuyla yuzyuze geleceğini göstermektedir Bu ise herkesin, bir biçimde, olumcul hastalık tanısı almış en azından bir kişiyle yakın ilişkide olacağı anlamını taşımaktadır Yaşamı tehdit eden bu yaşantıda hastadoktor iletişimi çok önemlidir Bu durum özellikle kotu haberin verildiği anlarda belirginleşir, hasta ve ailesinin iyilik halını etkiler Kotu haberin verilmesi, içerik, hasta - hekim iletişiminin özellikleri, hastaların tepkileri, böyle durumlarla nasıl başa çıkacağını bilememe vb nedenlerle hekimler için de zor bir görevdir Klinik deneyimler ve literatür bilgilen kotu haberin verilmesinin kotu biçimde yapılmasında, suçlanma duygusu, duyguların nasıl ifade edilmesi gerektiğini bilememe, soruların yanıtlarını bilememe ve kişisel olarak hastalıktan ve ölümden korkmanın etkili olduğu yönündedir Literatür bilgileri, uygulamada hekimlerin kotu haberin verilmesine ilişkin farklı tutumları olduğu yönündedir Bu tutumlar, hiç bir Şey hissetmeden ve korlemesıne, nazik ve uzuntulu bir tavırla ya da anlayışla ve olumlu yaklaşarak davranma olarak belirtilebilir Literatürde, kotu haberin, bu haberi alan kişiyi düşünerek, dikkat ederek ve ona saygs gösterip, empatı yapacak duyarlılıkla verilmesinin uygun olacağı ilen sürülmektedir Son sıralarda Batılı ülkelerde tıp eğitiminde konuyla ilgili iletişim becerilerinin kazanılmasının onemı bu konudaki eğitimin azlığı vurgulanmaktadır Bu konu ülkemizde de yeni yeni gündeme gelmekle birlikte, profesyonellerin konuya yeteri kadar eğildiklerini söylemek guçtur
