Factors Associated With Complex Regional Pain Syndrome Type 1 After Distal Radius Fracture: A Retrospective Case–Control Study

Purpose: Complex regional pain syndrome type 1 (CRPS-1) is a debilitating clinical disorder. Although recent studies have provided increasing evidence regarding the pathophysiology of CRPS-1, the exact mechanisms remain unclear. Therefore, this study aimed to identify risk factors associated with CPRS-1 after non-operatively treated distal radius fractures. Methods: This retrospective case-control study included 24 female patients diagnosed with CRPS-1 following distal radius fracture and 24 female patients without a CRPS-1. Data on patients' age, duration of immobilization, pre-existing diseases before the fracture were recorded. Multivariate logistic regression analysis was performed to identify risk factors associated with CRPS-1. Results: Data from 48 female patients were analyzed. The CRPS-1 group was significantly older (57.7 ± 11.7 vs 50.3 ± 11.3 years, p=0.033). The number of patients with psychiatric disorders was higher in the CRPS-1 group (41.7%) vs 8.3%, p=0.008). Immobilization duration was longer in the CRPS-1 group (p=0.035). Multivariate analysis showed that psychiatric disorders significantly increased the risk of CRPS-1 (OR: 6.68 [95% CI 1.12-39.68 p=0.037]). Age, diabetes mellitus, and duration of immobilization were not statistically significant independent predictors of CRPS-1 development (p=0.142, p=0.892, p=0.139, respectively). Conclusion and Suggestions: Our results demonstrate that patients with psychiatric disorders have a higher risk of developing CRPS type I following DRF. Careful monitoring of these patients for CRPS-1 may provide opportunities for early preventive and therapeutic interventions, thereby reducing disability.
