Yenidoğan Yoğun Bakım Ünitesinde Çalışan Hemşirelerin Bebeklerde Ağrı Yönetimine Yaklaşımları

Öz

Amaç: Bu çalışma, Yenidoğan Yoğun Bakım Ünitesinde (YYBÜ) çalışan hemşirelerin yenidoğan ağrısına ilişkin bilgi düzeylerinin, ağrı yönetimi yaklaşımları üzerindeki etkisini belirlemek amacıyla yapılmıştır. Gereç ve Yöntem: Tanımlayıcı-kesitsel tipte yürütülen çalışmada başlangıçta 90 hemşire değerlendirilmiş; katılmayı kabul etmeme ve eksik veri nedeniyle 12 hemşire çalışma dışı bırakılmıştır. Araştırma 78 hemşire ile tamamlanmıştır. Veriler, hemşirelerin sosyodemografik özellikleri ile yenidoğanda ağrı değerlendirme ve yönetimine ilişkin bilgi ve uygulamalarını içeren yapılandırılmış veri toplama formu aracılığıyla toplanmıştır. Veriler SPSS programı kullanılarak analiz edilmiş; tanımlayıcı istatistikler sayı, yüzde, ortalama ve standart sapma olarak sunulmuştur. Bulgular: Hemşirelerin büyük çoğunluğu genç, kısa deneyimli ve ağrı yönetimi konusunda eğitim almamış olup, en sık kullandıkları yöntemler emzik ve pozisyon verme (%92,3) ile ağrılı işlemlerin azaltılması ve masajdır (%80,8). Dokunma (%65,4) ve kanguru bakımı (%50) orta düzeyde tercih edilirken, şekerli solüsyon kullanımı düşük (%26,9) ve bu konuda bilgi eksikliği yüksektir (%73,1). Müzik en az tercih edilen yöntemdir (%11,5). Hemşirelerin çoğu (%73,1) ağrı yönetiminde farmakolojik ve non-farmakolojik yöntemleri birlikte kullanmayı tercih etmektedir. Sonuç: YYBÜ’de hemşirelerin yenidoğan ağrısına ilişkin bilgi düzeyleri, ağrı yönetimi uygulamalarını etkilemektedir. Kanıta dayalı rehberlerin klinik uygulamaya etkin biçimde yansıtılabilmesi için düzenli hizmet içi eğitimlerin planlanması ve uygulama engellerinin azaltılması önerilmektedir.

Approaches of Nurses Working in the Neonatal Intensive Care Unit to Pain Management in Infants

Öz

Objective: This study aimed to determine the impact of knowledge levels regarding neonatal pain among nurses working in the Neonatal Intensive Care Unit (NICU) on their pain management approaches. Materials and Methods: This descriptive cross-sectional study initially evaluated 90 nurses, of whom 12 were excluded due to non-participation and incomplete data. The study was completed with 78 nurses. Data were collected using a structured data collection form that included nurses’ sociodemographic characteristics and their knowledge and practices related to neonatal pain assessment and management. Data were analyzed using SPSS software, and descriptive statistics were presented as numbers, percentages, means, and standard deviations. Results: Most nurses were young, had limited professional experience, and had not received training in pain management. The most commonly used methods were pacifier use and positioning (92.3%), followed by reducing painful procedures and massage (80.8%). Touch (65.4%) and kangaroo care (50%) were moderately preferred, whereas the use of sweet solutions was low (26.9%) and 73.1% of the nurses were unaware that this method could be used. Music was the least preferred method (11.5%). The majority of nurses (73.1%) preferred to use both pharmacological and non-pharmacological methods in pain management. Conclusion: Nurses’ knowledge of neonatal pain in the NICU influences their pain management practices. To ensure that evidence-based guidelines are effectively reflected in clinical practice, regular in-service training programs should be implemented and barriers to practice should be reduced.
