Steroid kullanımına bağlı ortaya çıkan mani olgusu

Öz

Otoimmün hastalıklar başta olmak üzere birçok hastalığın tedavisinde kullanılan kortikosteroidlerin farklı yan etkileri vardır. Psikiyatrik yan etkilerinin patofizyolojisi hala tartışmalı olmakla birlikte bu konuda tespit edilmiş değişik risk etmenleri vardır. Kortikosteroid kullanımı sonrası gelişen mani tablosunun tedavisinde öncelikli olan steroidin kesilmesidir. Tedavide tek başına mizaç dengeleyici kullanılabileceği gibi, nöroleptik/mizaç dengeleyici birlikte de kullanılabilmektedir. Periferik yüz felci geçirmiş olan olgumuzda kortikosteroid kullanımı sonrasında ortaya çıkan psikotik özellikli mani tablosunun risk etmenleri, olanzapin tedavisine yanıtı ve seyri açısından tartışılması amaçlanmıştır.

A case of steroid- induced mania

Öz

Corticosteroids, used in the treatment of many illnesses, mainly autoimmune diseases, have various side effects. Although pathophysiology of psychiatric side effects are still controversial, there are different risk factors identified in this regard. Priority in the treatment of corticosteroid induced mania is to cut off steroids. As only mood stabilizer can be used, neuroleptic/mood stabilizer combinations also can be used in the treatment. In our case who had peripheral facial paralysis, corticosteroid induced mania with psychotic symptoms is aimed to discuss in terms of risk factors, response to olanzapine and treatment course.
