Quality of life in multiple sclerosis patients: impact of depression and physical limitations?

Öz

Aim: Quality of life (QOL) may be effected by disability. The aim of the present paper was to evaluate the depression which was common and treatable burden of multiple sclerosis (MS) and the effect of depression on quality of life. Method: Depression was evaluated in 62 MS patients. Data on health-related QOL (SF-36) was obtained in all MS patients. The results were compared to 24 ages and sex matched healthy controls. Disease related QOL and effects of depression, disability, and disease duration on QOL were evaluated in MS cases. Results: In the present study, 34% of the patients (n: 20) and 12.5% of the controls (n: 3) had depression (p<0.01). Sub scores of SF-36 mostly were lower statistically in patients (p<0.05). We found that there was a negative correlation between Beck depression scores and SF-36 sub scores (p<0.01). Functional limitations (disability scores) and disease duration was not related to SF-36 sub scores in statistical analysis. Conclusions: The presence of depression was found as a factor effecting QOL in comparison to disability or disease duration. It was concluded that evaluation and treatment of depression was important factor effecting QOL in follow-up of MS patients.

Multipl skleroz olgularında yaşam kalitesi: Fiziksel özürlülük ve depresyonun etkisi?

Öz

Amaç: Sağlıkla ilişkili yaşam kalitesi (YK) fiziksel özürlülüğe bağlı olarak etkilenir. Bu çalışmanın amacı multipl skleroz’un (MS) yaygın ve tedavi edilebilir bir boyutu olan depresyonun YK üzerine etkilerini ortaya koymaktır. Yöntem: Takipte olan 62 MS hastası, depresyon varlığı açısından değerlendirildi. Çalışmaya alınan tüm MS olguları için SF-36 YK ölçeği skoru hesaplandı. Yaş ve cinsiyet açısından benzer 24 sağlıklı kontrole ait depresyon ve YK ölçekleri sonuçlarıyla karşılaştırıldı. MS olgularında depresyonun, fiziksel özürlülüğün ve hastalık sürecinin YK üzerine etkileri araştırıldı. Bulgular: MS hastalarının %32.3 (n:20)’nde depresyon saptanırken, kontrol hastalarının yalnızca %12.5 (n:3)’nde depresyon saptandı (p<0.01). SF-36 ölçeği alt skorlarının birçoğunda da MS hastalarına ait skorlar kontrollere kıyasla istatistiksel olarak anlamlılık ifade edecek biçimde düşük bulundu (p<0.05). İlişki (korelasyon) analizleri sonrasında Beck depresyon skorları ve SF-36 alt ölçeklerinin tümüne ait skorlar arasında istatistiksel olarak anlamlı negatif bir ilişki bulundu (p<0.01). Fiziksel özürlülük ve hastalık süreci ile SF-36 alt ölçekleri skorlarının hiçbirisi arasında istatistiksel olarak anlamlı bir ilişki bulunamadı. Tartışma ve Sonuç: Fiziksel özürlülük veya hastalık sürecinden daha çok depresyonun varlığı YK’yı etkileyen bir etmen olarak bulundu. MS hastalarının takibinde depresyonun tespiti ve tedavisinin YK’ye katkısının önemli olduğu sonucuna varıldı.
