Neonatal Polycythemia: Clinical Features, Management, and Outcomes from a Turkish Single-Center Cohort

Objective: To describe risk factors, clinical manifestations, management, and outcomes of neonatal polycythemia in a large Turkish single-center cohort and to integrate these findings with recent literature. Material and Methods: Retrospective analysis of 99 neonates admitted between May 2006 and December 2007. Results: Small for gestational age (SGA, 24.2%) and large for gestational age (LGA, 15.2%) were major risk factors. Symptoms occurred in 73.7%, most commonly plethora (48.5%), gastrointestinal signs (20.2%), and central nervous system involvement (12.1%). Partial exchange transfusion (PET) was performed in 36.3%. Follow-up showed mild developmental delays (9–25%), without a significant difference between management groups. Conclusion: Neonatal polycythemia remains clinically important with consistent risk factors and manifestations. This cohort supports guideline-based conservative-first management, selective PET, and structured follow-up.

Yenidoğan Polisitemisi: Klinik Özellikler, Yönetim ve Sonuçlar – Tek Merkezli Türk Kohortu

Öz

Amaç: Yenidoğan polisitemisinin risk faktörlerini, klinik bulgularını, tedavi yaklaşımlarını ve sonuçlarını büyük bir Türk tek merkez kohortunda tanımlamak ve güncel literatürle karşılaştırmak. Gereç ve Yöntem: Mayıs 2006 – Aralık 2007 arasında yatırılarak izlenen 99 yenidoğan retrospektif olarak değerlendirildi. Bulgular: En sık risk faktörleri gestasyonel yaşa göre küçük olma (SGA, %24,2) ve gestasyonel yaşa göre büyük olma (LGA, %15,2) idi. Olguların %73,7’si semptomatikti; en sık pletora (%48,5), gastrointestinal bulgular (%20,2) ve santral sinir sistemi bulguları (%12,1) görüldü. Parsiyel değişim transfüzyonu (PET) %36,3 olguda uygulandı. 12–24 aylık takipte hafif gelişimsel gecikmeler izlendi (%9–25); PET ve konservatif yönetim grupları arasında anlamlı fark saptanmadı. Sonuç: Yenidoğan polisitemisi klinik açıdan önemli bir durum olmaya devam etmektedir. Bulgular konservatif yaklaşımın öncelikli olduğunu, PET’in seçilmiş olgularda uygulanması gerektiğini ve düzenli takip ihtiyacını desteklemektedir.
