Geç Tanı Konulan Konjenital Boyun Kütlesi: Yaşlılarda Tiroglossal Kanal Kisti

Öz

Tiroglossal kanal kistleri (TGDK), çocuklar ve genç erişkinlerde sık görülen doğumsal boyun anomalileridir; 60 yaş üstünde nadiren görülür ve yalnızca %0,6 oranında rastlanır. Submental bölgede kitle ile başvuran ve TGDK ile aynı zamanda prostat karsinomu tanısı alan 69 yaşındaki bir erkek hastayı tanımlamaktadır. Kist, kanalı ve hyoid kemiğin bir kısmının çıkarılmasını içeren Sistrunk prosedürü ile tedavi edilmiştir. Ameliyat sonrası altı aylık takipte nüks veya komplikasyon izlenmemiştir. Bu olgu, TGDK'nin ileri yaşlarda nadir görülen bir sunumunu ve görüntüleme yöntemlerinde diğer patolojilerle karışabileceğini vurgulamaktadır. Doğru tanı ve tedavisi, klinik tecrübe ve intraoperatif değerlendirme gerektirir. Bu vakada metastaz saptanmamış olsa da, kanser öyküsü bulunan hastalarda malignite olasılığı göz önünde bulundurulmalıdır.

Delayed Diagnosis of a Congenital Neck Mass: Thyroglossal Duct Cyst in the Elderly.

Öz

Thyroglossal duct cysts (TGDCs) are congenital neck anomalies commonly seen in children and young adults, with rare occurrence in individuals over 60, accounting for only 0.6% of cases. This report describes a 69-year-old male presenting with a submental mass, later diagnosed as TGDC alongside prostate carcinoma. The patient underwent a Sistrunk procedure involving excision of the cyst, duct, and part of the hyoid bone. At six months postoperatively, no recurrence or complications were observed. This case highlights the rare late-life presentation of TGDCs, which may mimic other pathologies on imaging. Accurate diagnosis and management require clinical expertise and intraoperative judgment. While no metastasis was detected here, malignancy should be considered in patients with a history of cancer.
