İleostomili Total Kolonik Aganglionozisli Hastanın Gordon’un Fonksiyonel Sağlık Örüntüleri Modeline Göre Hemşirelik Bakımı: Olgu Sunumu

Öz

Bu olgu sunumu Hirschsprung Hastalığı’nın nadir görülen bir çeşidi olan Total Kolonik Aganglionozis şüphesi nedeniyle ileostomi açılan bebeğin hemşirelik bakım planının sunulması amacıyla yapılmıştır. Veri toplamada Gordon’un Fonksiyonel Sağlık Örüntüleri Modeli kullanılmıştır. Veriler, hastane bilgi sistem kayıtları, tıbbi tedavi ve hemşire gözlem kayıtları, ebeveynlerle birebir görüşme yöntemi ile toplandı, modele göre bakımı planlandı, uygulandı ve değerlendirildi. Bebek ve annesinin gereksinimine göre bakım sürecinde; bakım verici rolünde zorlanma, aile içi süreçlerde bozulma, sözel iletişimde bozulma, beden gereksiniminden az beslenme/ malnütrisyon, aktivite intoleransı, elektrolit dengesizliği, doku bütünlüğünde bozulma riski, sıvı elektrolit eksikliği riski ve enfeksiyon riski hemşirelik tanıları konulmuştur. Sistematik bir yaklaşımla değerlendirilen olgumuzda, hasta ve ailesi biyopsikososyal olarak değerlendirilmiş ve bütüncül bir bakış açısıyla hemşirelik tanıları ile girişimler açıklanmıştır.

Nursing Care of a Patient With Ileostomy Total Colonic Aganglionosis According to Gordon's Functional Health Patterns Model: A Case Report

Öz

This case report was conducted present the nursing care plan of the baby who underwent ileostomy, because the suspicion of Total Colonic Aganglionosis, which is a rare type of Hirschsprung's Disease. Gordon's Functional Health Patterns Model was applied in data collection. Data, hospital information system records, medical treatment and nurse observation records were collected by one-to-one interviews with parents. Therefore care was planned, implemented and evaluated according to the model. In the care process according to the needs of the baby and its mother; Nursing diagnoses were made for difficulties in the caregiver role, impaired verbal communication, poor nutrition / malnutrition, activity intolerance, electrolyte imbalance, risk of tissue integrity, fluid electrolyte deficiency and infection risk. In our case, which was evaluated with a systematic approach, the patient and patient’s family were evaluated biopsychosocially and nursing diagnoses and interventions were explained with a holistic perspective.
