Sensitivity and Treatment in Molar Incisor Hypomineralization (MIH) Teeth

Abstract

Molar Incisor Hypomineralization (MIH) is a developmental enamel defect characterized by hypomineralization and discoloration of one or more permanent first molars and incisors, often leading to sensitivity, enamel breakdown, and an increased risk of cavities. In MIH-affected teeth, the enamel tissue has reduced mineral content, increased protein and water content, and higher porosity. As a result of these physicochemical changes, negative clinical outcomes such as hypersensitivity, difficulty in achieving adequate anesthesia, atypical carious lesions, aesthetic concerns, and reduced quality of life arise. One of the most common and challenging complaints encountered by patients with MIH, which significantly impacts the treatment process, is hypersensitivity. This condition greatly reduces the quality of life, negatively affecting behaviors related to eating, drinking, and oral hygiene. Managing hypersensitivity in MIH-affected teeth can range from preventive and non-invasive approaches to invasive procedures and extractions due to the wide variety of treatment methods and the significant differences in the severity of the affected teeth. The aim of this review is to provide clinicians with a different perspective by comparing the current literature on the treatment of hypersensitivity in MIH-affected teeth.

Abstract

Büyük Azı Keser Hipomineralizasyonu (BAKH), en az bir veya daha fazla birinci daimî molar diş ile keser dişleri etkileyen, minede hipomineralizasyon ve renk değişikliği ile karakterize, sıklıkla hassasiyet, mine yıkımı ve artan çürük riski ile sonuçlanan gelişimsel bir mine defektidir. BAKH'lı dişlerde mine dokusu, azalmış mineral, artmış protein ve su içeriği ile artmış poroziteye sahiptir. Bu fizikokimyasal değişikliklerin sonucu olarak aşırı hassasiyet, yeterli anestezi sağlamakta zorluk, atipik çürük lezyonları, estetik kaygılar ve yaşam kalitesinde azalma gibi olumsuz klinik sonuçlar ortaya çıkmaktadır. BAKH'lı dişlerde hastaların en sık karşılaştığı ve tedavi sürecini en fazla zorlayan şikayetlerden biri aşırı hassasiyettir. Bu durum, hastaların yaşam kalitesini önemli ölçüde düşürerek, özellikle yemek yeme, içecek tüketimi ve ağız hijyenine yönelik davranışları üzerinde olumsuz etkiler yaratır. BAKH'lı dişlerde aşırı hassasiyetin yönetimi, tedavi yöntemlerinin geniş bir yelpazeye yayılması ve etkilenen dişlerin şiddetinin büyük ölçüde farklılık göstermesi nedeniyle, koruyucu ve invaziv olmayan yaklaşımlardan, invaziv işlemler ve diş çekimlerine kadar uzanabilmektedir. Bu derlemenin amacı, MIH'lı dişlerde görülen aşırı hassasiyetin tedavisine yönelik güncel literatürde yer alan bilgileri karşılaştırarak tartışmak ve klinisyenlere bakış açısı kazandırmaktır.
