Kronik Seröz Otitis Media Olan Çocuk Hastalarda Sistemik İnflamatuar İndeks Değerleri

Öz

Amaç: Kronik efüzyonlu otitis media, çocuklarda adenotonsil hastalıklarına eşlik edebilen bir hastalıktır. Tekrarlayan ve takiplerinde düzelme saptanmayan kronik efüzyonlu seröz otitis media gelişen hastalarda, adenotonsillektomi operasyonuna ek olarak ventilasyon tüpü tatbiki de yapılmaktadır. Bu çalışmada, adenotonsillektomi planlanan hastalarda kulak zarına ventilasyon tüpü tatbiki endikasyonu ile sistemik inflamatuar belirteçler arasında bir ilişki varlığının değerlendirilmesi amaçlanmıştır. Materyal ve metot: Hastalar adenotonsillektomi operasyonu yapılan ve adenotonsillektomi ile birlikte ventilasyon tüpü tatbiki yapılan hastalar olarak iki gruba ayrıldı. Tüm hastaların tam kan sayımı sonuçlarından platelet, nötrofil ve lenfosit sayıları kullanılarak sistemik inflamatuar indeks (Sİİ) değerleri hesaplandı. Bulgular: Sİİ değerleri grup 1’ de 481 ± 254 (x109/l) ve grup 2’ de 490 ± 290 (x109/l) idi (p= 0,74). Kronik seröz otitis media saptanan çocuk hastalar ile saptanmayan hastalar arasında nötrofil, lenfosit, platelet ve Sİİ değerleri açısından istatiksel olarak anlamlı bir fark saptanmadı (p= 0,89; p= 0,97; p= 0,78; p= 0,74). Sonuç: Çalışmamızda adenotonsil hipertrofisi olan hastalarda kronik seröz otitis media varlığının tam kan sayımı parametreleri ve Sİİ’de istatiksel olarak anlamlı bir değişime neden olmadığı görüldü. Adenotonsillektomi planlanan hastalarda ventilasyon tüpü uygulamasının endikasyonunda sistemik inflamatuar indeksin prognostik öneminin daha iyi anlaşılması için daha büyük örneklem büyüklüğüne sahip çalışmalara ihtiyaç vardır

The Systemic Inflammatory Index Values Of Pediatric Patients With Chronic Serous Otitis Media

Öz

Objective: Otitis media with chronic effusion is a disease that can accompany adenotonsillary diseases in children. In patients who develop serous otitis media with chronic effusion, which recurs and does not improve during follow-up, ventilation tube application can be performed in addition to the adenotonsillectomy operation. In this study, it was aimed to evaluate the existence of a relationship between the indication of ventilation tube application to the eardrum and systemic inflammatory markers in patients planned for adenotonsillectomy. Material and Methods: The patients were divided into two groups: those who underwent adenotonsillectomy and those who underwent ventilation tube application along with adenotonsillectomy. Systemic inflammatory index (SII) values were calculated using platelet, neutrophil and lymphocyte counts from the complete blood count results of all patients. Results: SII values were 481 ± 254 (x109/l) in group 1 and 490 ± 290 (x109/l) in group 2 (p= 0.74). The neutrophil, lymphocyte, platelet counts and SII levels did not differ between patients with and without chronic serous otitis media in addition to adenotonsillary hypertrophy (p= 0.89; p= 0.97; p= 0.78; p= 0.74). Conclusion: In our study, it was observed that the presence of chronic serous otitis media did not cause a statistically significant difference in complete blood count parameters and SII in patients with adenotonsillar hypertrophy. Further studies with a larger sample size may help to show the prognostic importance of the SII in the indication of ventilation tube application in patients planned for adenotonsillectomy
