Orbital Apex Syndrome Due To Invasive Aspergillosis: Case Report

Tissue invasion due to Aspergillus is most commonly seen in the paranasal sinuses in the head and neck region. Invasive aspergillosis is an opportunistic infection and is frequently seen in immunosuppressed patients. It can spread to the orbit by causing bone erosion, and to the cavernous sinus and intracranial area via vascular access due to angioinvasion caused by opportunistic infection. A 60-year-old female patient complained of pain in the right half of the face that had not gone away for three months. Paranasal sinus tomography revealed a defect in the anterior and lateral walls of the right maxillary sinus. Punch biopsies were taken from the polypoid necrotic tissue filling the maxillary sinus, and the pathology result was reported as aspergillosis. Following voriconazole treatment, in the first year after surgery, II, III, IV, V2, VI. We performed right optic nerve decompression + functional endoscopic sinus surgery on the patient who was diagnosed with cranial nerve paralysis. The infection invaded the lamina papyrea, anterior to the skull base, and lateral to the sphenoid sinus. The patient was discharged with Posaconazole oral therapy after 14 days of IV amphotericin B treatment

İnvaziv Aspergilloza Bağlı Gelişen Orbital Apeks Sendromu: Olgu Sunumu

Öz

Aspergillusa bağlı doku invazyonu baş boyun bölgesinde en sık paranazal sinüslerde görülmektedir. İnvaziv aspergilloz, fırsatçı enfeksiyondur ve sıklıkla immünsüprese hastalarda görülmektedir. Kemik erozyonuna yol açarak orbitaya, fırsatçı enfeksiyonun neden olduğu anjioinvazyon sebebiyle vasküler yolla kavernöz sinüse ve intrakraniyal alana yayılabilir. 60 yaşında kadın hasta, üç aydır geçmeyen yüzün sağ yarımında ağrı şikayeti mevcuttu. Paranazal sinüs tomografisinde sağ maksiller sinüs anterior ve lateral duvarında defekt izlendi. Maksiller sinüsü dolduran polipoid nekroze dokudan punch biyopsiler alındı, patoloji sonucu aspergillozis olarak raporlandı. Vorikonazol tedavisinin ardından, cerrahi sonrası birinci yılında, II, III, IV, V2, VI. Kranial sinir paralizileri tespit edilen hastaya, tarafımızca sağ optik sinir dekompresyonu + fonksiyonel endoskopik sinüs cerrahisi yapıldı. Enfeksiyon lamina papriseaya, kafa tabanı önüne ve sfenoid sinüs lateraline invazeydi. Hastaya 14 gün İV amfoterisin B tedavisinin ardından Posakonazol oral tedavi ile taburcu edildi.
