Adenoid Cystic Carcinoma of the External Auditory Canal: A Case Report

The external auditory canal can present with diverse pathologies, including infections, osteomas, cerumen impaction, dermatological conditions, traumatic lesions, and malignancies. Among malignant tumors, squamous cell carcinoma, basal cell carcinoma, and malignant melanoma are the most frequent, while sweat gland-derived tumors, although rare, should also be considered in the differential diagnosis. These tumors may resemble infectious diseases, causing diagnostic delays, and only a few cases of sweat gland malignancies localized to the external auditory canal have been reported in the literature. Their potential for local invasion and recurrence underscores the importance of early diagnosis and proper surgical treatment. We report a 47-year-old female with adenoid cystic carcinoma of the external auditory canal. Despite its slow progression, this rare tumor demonstrates a marked tendency for perineural invasion and long-term recurrence. Such features make accurate diagnosis and careful management crucial, particularly in anatomically restricted areas like the external auditory canal.

Dış Kulak Yolunda Adenoid Kistik Karsinom: Olgu Sunumu

Öz

Dış kulak yolunda görülen patolojiler oldukça çeşitlidir. En sık lokal enfeksiyonlar, osteomlar, serumen tıkanıklıkları, dermatolojik hastalıklar, travmatik lezyonlar ve maligniteler görülür. Maligniteler arasında skuamöz hücreli karsinom, bazal hücreli karsinom ve malign melanom öne çıkarken nadir de olsa ter bezi kökenli malign tümörler de ayırıcı tanıda dikkate alınmalıdır. Bu tümörler sıklıkla enfeksiyon benzeri bulgularla seyrettiğinden tanıda gecikmeye yol açabilmektedir. Literatürde dış kulak yoluna lokalize ter bezi malign tümörleri nadiren bildirilmiştir. Lokal invazyon ve nüks eğilimleri nedeniyle erken tanı ve cerrahi tedavi klinik açıdan önemlidir. Bu çalışmada, 47 yaşında kadın hastada dış kulak yolunda adenoid kistik karsinom olgusu sunulmuştur. Yavaş seyirli olmasına rağmen perinöral invazyon ve uzun dönemde nüks riski yüksek olan bu nadir tümör, dış kulak yolu gibi sınırlı alanlarda tanı ve tedavide özel dikkat gerektirir.
