YOĞUN BAKIM ÜNİTESİNDE EL HİJYENİ KÜLTÜRÜNÜN İNCELENMESİ

AMAÇ: Hastanelerde el hijyeni uygulamaları, hasta bakım kalitesinin iyileştirilmesinde önemli bir role sahiptir. El hijyeni uygulamalarının yetersiz olması, özellikli birimlerde (Yoğun bakım ünitesi) (YBÜ) ve klinikler içinde-klinikler arasında çapraz bulaşa sebep olur ve bu da hastane kaynaklı enfeksiyon insidansında artışa yol açar. Bu duruma engel olmak amacıyla, ilgili personelin el hijyeni konusunda bilgilendirilmesi ve bilinç kazanması büyük önem taşır. Bu çalışma ile yoğun bakım ünitelerinde (YBÜ) çalışan sağlık personelinin el hijyeni bilincinin değerlendirilmesi ve farkındalık oluşturulması amaçlanmıştır. YÖNTEM: El hijyeni kültürünü belirlemek üzere; 20 sürüntü YBÜ personelinin ellerinden, 31 sürüntü de YBÜ içerisindeki fiziki alanlardan olmak üzere toplam 51 sürüntü örneği çalışmaya alınmıştır. Bu sürüntü örneklerinin değerlendirilmesi, laboratuvarda klasik temel bakteri tanımlama yöntemine göre yapılmıştır. Bu iş için; sürüntü örneklerinin 2 farklı besiyerine (Kanlı agar ve EMB agar) ekimi yapılmış, üreyen kolonilerden biyokimyasal özellikler test edilerek tanımlama (identifikasyon) yapılmıştır. BULGULAR: 20’si personel el sürüntüleri, 31’i cansız yüzey sürüntüleri olmak üzere 51 sürüntü örneği çalışılmış olup; toplam 15 örnekte üreme görülmüştür. Üreme olan 15 örneğin; 9’u personel kaynaklı olup, 6’sı cansız yüzeylerden alınan sürüntü örneklerinden oluşmaktadır. Üreme sonuçlarına göre; bu 15 üremenin 9’unda (% 60) Koagülaz Negatif Stafilokok (KNS) varlığı gösterilmiştir. KNS’yi, S. aureus (3), E.coli (1), Proteus spp. (1), Acinetobacter spp. (1) takip etmekteydi. SONUÇ VE ÖNERİLER: YBÜ çalışanlarının el hijyeni bilincini ve YBÜ içindeki kontamine alanları görebilmek adına yapılan bu çalışmanın sonuçlarına göre alınabilecek önlemler; El hijyeni uygulamalarına yönelik YBÜ kontrol listelerinin (check-list) oluşturulması ve güncellenmesi, hasta bakım alanında kullanılan dezenfektanlar ve sıvı/köpük sabunlardan periyodik kontaminasyon taraması amaçlı bakteriyel kültür takibi, sağlık personelinin kişisel sağlık durumunun takibi ve el hijyeni eğitimlerinin ve uygulamalarının sürdürülebilir olması gerektiğini düşünüyoruz. Inadequate hand hygiene practices cause cross-contamination in specialized units (Intensive care unit) (ICU) and within-clinics, leading to an increased incidence of hospital-acquired infections. In order to prevent this situation, it is of great importance to inform and raise awareness of the relevant personnel about hand hygiene. Whith this study, it was aimed to evaluate the hand hygiene awareness of health personnel working in ICU and raise awareness. METHODS: In order to determine hand hygiene culture; A total of 51 swab samples, 20 from hands of the ICU staff and 31 from the physical areas in the ICU, were included in the study. Evaluation of these swab samples was made in the laboratory according to the classical basic bacteria identification method. For this work; swab samples were cultivated on 2 different media (Blood agar and EMB agar), biochemical properties were tested from the growing colonies and identification was made. RESULTS: 51 swab samples, 20 of which were personnel hand swabs, 31 of which were inanimate surface swabs, were studied; Reproduction was observed in a total of 15 specimens. For example, 15 breeding; It consists of swab samples, 9 of which are from personnel and 6 of which are taken from inanimate surfaces. According to the reproductive results; Coagulase Negative Staph in 9 (% 60) of these 15 growths. (KNS) presence is shown. KNS, S. aureus (3), E.coli (1), Proteus spp. (1), Acinetobacter spp. (1). CONCLUSİON AND RECOMMENDATİONS: The precautions that can be taken according to the results of this study, which was carried out in order to see hand hygiene awareness of the ICU staff and the contaminated areas in the ICU: It is necessary to create and update ICU checklists for hand hygiene practices, bacterial culture monitoring for periodic contamination screening from disinfectants and liquid (foam) soaps used in patient care area, monitoring personal health status of healthcare personnel and We think that hand hygiene training and practices should be sustainable.
