Subungual exocytosis is a common benign bone tumor. It occurs in the distal terminal phalanx and is seen as pink nodules under the free edge of the nail bed. It is common in adolescents and young adults. The lesion becomes symptomatic with mechanical irritation or growth. Pain, nail elevation, deformity and infection can occur. It is mostly found in the first toe. A 16-year-old male patient presented to our clinic with complaints of swelling, pain, and disfigurement of the right foot first toe. On dermatological examination, there was edema, erythema, nail elevation, and a 3-4 mm nodule under the nail of the right foot first toe. Exophytic bone growth was seen in the medial distal phalanx on two-way graphs of the right foot first toe. The lesion was diagnosed as subungal exocytosis and excised by plastic surgery after treatment of the infection. We present our case to discuss the differential diagnosis of subungual exocytosis and emphasize that it can be diagnosed easily
