Assessment of eccentric coordination, endurance, and muscle strength in patients with patellofemoral pain syndrome

Öz

Purpose: Aim of this study was to investigate muscle strength, endurance and coordination of
the knee in patients with patellofemoral pain syndrome (PFPS). Materials and methods:
Thirty five women with unilateral PFPS and twenty healthy women participated in the study.
Pain levels during ascending and descending stairs and squatting, were evaluated using
numeric pain scale. The Quadriceps and Hamstring isokinetic muscle strength were assessed at
the velocity of 60º/sec and 180º/sec. Monitored Rehab Systems-Functional Squat System was
used for coordination and endurance evaluation. For functional status of the patients Kujala
Patellofemoral Score was used. Results: The most severe pain was recorded during squatting
in the PFPS group. Quadriceps Femoris (p=0.01) and Hamstring (p=0.04) isokinetic muscle
strength results were better in the healthy group compared to PFPS group in both effected and
non-effected sides. Significant difference was found in PFPS group’s eccentric coordination in
effected and non-effected sides (p=0.01). Endurance results were better in the dominant side
of the healthy group when compared with the effected side of the PFPS group (p=0.01). Kujala
score was 58.17±16.55 in PFPS group. Conclusion: In addition to decreased muscle strength
and functional status, eccentric coordination and endurance also change in PFPS.

Patellofemoral ağrı sendromunda eksentrik koordinasyon, işlevsel dayanıklılık ve kas kuvvetinin değerlendirilmesi

Öz

Amaç: Bu çalışma, patellofemoral ağrı sendromu (PFAS) tanısı konulan hastalarda etkilenen dize ait kas kuvveti, işlevsel dayanıklılık ve koordinasyon bulgularının araştırılması amacıyla planlandı. Gereç ve yöntem: Tek taraflı PFAS tanısı konulan 35 kadın hasta ve 20 sağlıklı kadın çalışmaya alındı. Merdiven inme ve çıkma ile çömelme hareketleri sırasındaki ağrı şiddeti sayısal ağrı ölçütü ile değerlendirildi. Quadriceps femoris ve Hamstring izokinetik kas kuvveti 60º/sn ve 180º/sn hızda ölçüldü. Koordinasyon ve işlevsel dayanıklılık testleri Monitörize Rehabilitasyon Sistemleri – İşlevsel Diz Bükme Sistemi kullanılarak değerlendirildi. Hastaların işlevsel seviyeleri Kujala patellofemoral ağrı skoru ile belirlendi. Sonuçlar: Hastaların en yüksek ağrı şiddeti çömelme sırasındaydı. Hastaların etkilenen ve etkilenmeyen ekstremitelerinin 60º/sn hızda değerlendirilen Quadriceps femoris (p=0.01) ve Hamstring (p=0.04) izokinetik kas kuvveti sonuçlarında sağlıklı olgular lehine anlamlı fark bulundu. Hastaların her iki ekstremitesi arasında eksentrik koordinasyonda anlamlı fark bulundu (p=0.01). Hastaların etkilenen tarafları ile sağlıklı kontrollerin dominant tarafları karşılaştırıldığında işlevsel dayanıklılık sonuçları arasında sağlıklı olgular lehine anlamlı fark bulundu (p=0.01). Hastaların Kujala patellofemoral ağrı skoru 58.17±16.55 olarak kaydedildi. Tartışma: PFAS’de kas kuvveti ve işlevsellikte azalmaya ek olarak, eksentrik koordinasyon ve işlevsel dayanıklılıkta da değişimler vardır
