Staphylococcus aureus suşlarında koagulaz testi için uygun sıcaklığın araştırılması

Öz

Amaç: Mikrobiyoloji laboratuvarında etüvün ısısı önemlidir. Bakterilerin üretilmesinde ve antimikrobiyal duyarlılığının saptanmasında genellikle önerilen etüv ısısı 35±2 OC’dir. Bununla birlikte Staphylococcus aureus suşlarında doğru bir şekilde metisilin direncinin saptanmasında 35 OC’nin üzerinde etüv ısısının uygun olmadığı, 35 OC’de metisilin direncinin doğru saptanamayacağı bilinmektedir. S. aureus’un tanımlanmasında kullanılan tüp koagulaz testinin yapılmasında önerilen etüv ısısı farklı kaynaklarda 35 OC, 37 OC ve 35 OC - 37 OC olarak belirtilmiştir. Bu çalışmada S. aureus suşlarında tüp koagulaz testi için en uygun etüv ısısı araştırılması amaçlanmıştır.Yöntemler: Bu çalışma hastanemiz Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji Laboratuvarında yapılmıştır. Bakterilerin tanımlanmasında VITEK 2 otomatize sistem kullanılmıştır. Çeşitli klinik örneklerden izole edilen 110 S. aureus suşu çalışmaya dahil edilmiştir. Tüp koagülaz testi için tavşan plazması kullanılmış, test üretici firma önerileri doğrultusunda yapılmıştır. Her bakteri için iki ayrı koagulaz tüpü hazırlanmış, tüpler ayrı ayrı 35 OC ve 37 OC’ye ayarlanmış iki farklı etüvde inkübe edilmiştir. Birinci, ikinci, üçüncü, dördüncü ve 24. saatin sonunda tüpler pıhtı oluşumu için değerlendirilmiştir. Sonuçlar üç grupta değerlendirilmiştir. Grup 1: Pıhtı oluşumu gözlenmeyenGrup 2: Zayıf pıhtı oluşumu gözlenen Grup 3: Güçlü pıhtı oluşumu gözlenenVerilerin analizi için SPSS 15.0 programı ve grupların karşılaştırılması için de McNemar Bowker testi kullanılmıştır.Koagulaz testi Staphylococcus aureus’u diğer stafilokok türlerinden ayırmada kullanılan önemli ve uygulaması kolay bir yöntemdir. Stafilokok olduğu düşünülen, Gram boyamasında gram olumlu koklar saptanan ve katalazı pozitif olan tüm izolatlarda yapılmalıdır. Koagulaz testi, Lam testi ve tüp testi olarak iki temel yöntem ile yapılır. Lam testi ile bağlı koagulaz kümeleştirme faktörü= clumping factor saptanmaktadır. Bu yöntemde; bakteri yüzeyindeki kümeleştirme faktorü, plazmadaki fibrinojeni pıhtılaştırıp stafilokokların kümelenmesine neden olur. Tüp testinde ise stafilokokların besiyerine saldıkları bağımsız koagulaz araştırılır. Bu enzim niteliğindeki madde, plazmada bulunan bir faktör ile koagulaz reaksiyon faktör ilişki kurar ve fibrinojeni pıhtılaştırır. Tüp testinde stafilokok olduğu düşünülen kolonilerden birkaç tane alınarak tüpe konulan plazma içinde ezilirerek emülsiyon haline getirilir. Tüpler 35 OC - 37 OC’lik etüve cam tüplere, ticari firmanın önerileri doğrultusunda yerleştirilir, birinci, ikinci, üçüncü ve dördüncü sulandırılmış tavşan plazmasından 0,5’er ml saatlerde pıhtı oluşup oluşmadığına bakılır 1 . Testin ilk dört saatlik uygulaması sırasında etüvün sıcaklık derecesinin farklı kaynaklarda 35 OC, 37 OC, 35 OC - 37 OC derece olarak belirtildiği gözlenmektedir . İlk dört saatte pıhtı gözlenmeyen tüplerde gecikmiş koagülaz aktivitesinin varlığını araştırmak için tüpün oda ısısında 24. saate kadar inkübe edilmesi gerekmektedir. Bu çalışmada S. aureus Bulgular: Suşların hepsi her iki etüv derecesinde de test süresince pıhtı oluşturmuştur. Bakterilerin zamana göre 35 OC ve 37 OC’de pıhtı oluşturma durumları değerlendirilmiştir. amacıyla Sonuçların istatistiksel inkubasyon sıcaklıkları ve gruplar arasında anlamlı fark saptanmamıştır. karşılaştırılması değerlendirmede yapılan Sonuç: Çalışmanın sonuçlarına dayanarak hem 35 OC ve hem de 37 OC’nin S. aureus suşlarında koagulaz saptanmasında uygun olduğu bulunmuştur

Investigation of optimum temperature for coagulase test in Stahylococcus aureus strains

Öz

Objective: Temperature of the incubator is important in microbiology laboratories. The recommended temperature is generally 35±2 OC for the detection of growing and antimicrobial susceptibility of bacteria. Nevertheless it is known that temperature over 35 OC is inappropriate for detecting methicillin resistance in Staphylococcus aureus. In different references, the optimum temperature for tube coagulase test used for differentiating S. aureus from other staphylococci is recommended as 35 OC, 37 OC, and 35 OC - 37 OC. In this study it was aimed to investigate the most appropriate incubator temperature for tube coagulase test in S. aureus strains. Methods: The study was conducted in Infectious Diseases Laboratory of our hospital. VITEK 2 automated system was used for identification of bacteria. Totally 110 S. aureus strains isolated from various clinical samples were included in the study. Rabbit plasma was used for tube coagulase test and the test was performed according to the manufacturer’s instructions. Two identical sets of tubes were prepared for each strain and each tube was incubated at 35 OC and 37 OC in different incubators. All the tubes were read at the end of first, second, third, fourth and 24th hour for clot formation. Results were evaluated in three groups. Group 1: No clot formation Group 2: Weak clot formation Group 3: Strong clot formation SPSS 15.0 program was used for data analysis and McNemar Bowker test was used for comparing groups.Results: All of the strains had clot formation at both of the incubator temperatures during test period. The clot formation degree of strains were examined at 35 OC and 37 OC according to time. The statistical analysis showed no significant differences between incubation temperatures and groups. Conclusion: According to the results obtained in this study, both 35 OC and 37 OC was found appropriate for determining coagulase positivity in S. aureus.
