Meniere Hastalığında Çok Disiplinli Yaklaşım: Odyoloji Perspektifinden Bir Olgu

Öz

Amaç: Geniş klinik özellikler sergileyen Meniere Hastalığı; değerlendirme, tedavi ve takip süreçlerinde farklı uygulama ve protokollerle ele alınmaktadır. Bu yazıda, 10 yıllık süreçte pek çok merkeze başvurmuş, farklı tedavi deneyimleri de bulunan, Meniere tanılı bir vakanın odyometrik, immitansmetrik ve vestibüler bulgular ışında değerlendirilmesi, semptomlarının seyri ve hastanın deneyimleri tartışılmaktadır. Yöntem: Meniere tanısı almış 48 yaşında kadın hasta odyoloji kliniğinde değerlendirilmiştir. Hastanın ayrıntılı öyküsü alınmış; saf ses odyometri, timpanometri ve akustik refleks ölçümleri yapılmış, ayrıca yatak başı vestibüler muayene gerçekleştirilmiştir. Bulgular: Saf ses odyometri sonucunda sağ kulakta hafif derecede sensörinöral işitme kaybı tespit edilmiştir. İmmitansmetrik değerlendirmede bilateral tip A timpanogram ve akustik refleksler normal bulunmuştur. Vestibüler muayenede periferik veya santral patoloji düşündürecek bulguya rastlanmamıştır. Hastanın öyküsünde masseter kaslara yapılan botulinum toksin uygulaması sonrası vestibüler yakınmaların azaldığı, ancak işitme kaybının devam ettiği öğrenilmiştir. Sonuç: Bu olguda temporomandibular eklem sorununa yönelik yapılan müdahalenin hastanın vestibüler sıkıntılarını büyük oranda gerilettiği, ancak işitme kaybının bundan bağımsız sorun olarak bildirildiği görülmekte. Meniere hastalığında yaşanan ataklar ve atakların özellikleri hastaların farklı merkezlerde çözüm aramasına neden olabilir. Ancak hastaların, hastalığın seyri ve tedavi seçenekleri ile ilgili bilinçlendirilmesinin önemli olduğu görülmektedir. Eşlik eden diğer sistem bozuklukları da titizlikle ele alınmalıdır. Hastanın daha bütüncül değerlendirilmesi, sadece semptomatik tedavi değil de altta yatan nedenlerin araştırıldığı genel sağlık durumunu iyileştirmeye odaklanılması, işitme performansının ayrıntı ele alınması, gerekli hallerde o anki ihtiyacına yönelik cihaz planlanması tartışılmıştır.

A Multidisciplinary Approach To Meniere’s Disease: A Case From The Audiology Perspective

Öz

Objective: Ménière’s disease, which presents with a wide range of clinical features, is approached with different protocols and practices during assessment, treatment, and follow-up. This report discusses the evaluation, symptom progression, and patient experience of a diagnosed Ménière’s case, who applied to multiple centers and underwent various treatment approaches over a 10-year period, based on audiometric, immittance, and vestibular findings. Method: A 48-year-old female patient diagnosed with Ménière’s disease was evaluated in the audiology clinic. A detailed medical history was obtained; pure-tone audiometry, tympanometry, and acoustic reflex measurements were performed, and additionally, a bedside vestibular examination was conducted. Results: Pure-tone audiometry revealed mild sensorineural hearing loss in the right ear. Immittance evaluation showed bilateral type A tympanograms and normal acoustic reflexes. Vestibular examination did not reveal any findings suggestive of peripheral or central pathology. The patient’s history indicated that vestibular complaints decreased after botulinum toxin injections into the masseter muscles; however, the hearing loss persisted. Conclusion: In this case, intervention targeting temporomandibular joint problems significantly alleviated the patient’s vestibular complaints, while the hearing loss persisted as an independent issue. The variability in attacks and their characteristics in Ménière’s disease may lead patients to seek care from multiple centers. However, it is crucial to raise patients’ awareness about the course of the disease and available treatment options. Coexisting systemic disorders should also be carefully considered. A more holistic evaluation of the patient, with a focus not only on symptomatic relief but also on identifying underlying causes and improving overall health, is essential. The case highlights the importance of thoroughly addressing hearing performance and planning hearing aid use when needed based on the current individual needs.
