EVALUATION OF THE RELATIONSHIPS AMONG MINDFULNESS, CLINICAL DECISION-MAKING, AND NURSING CARE COMPETENCE IN INTENSIVE CARE NURSES

Öz

Aim: This descriptive cross-sectional study was conducted to evaluate the relationships among nursing care competence, mindfulness, and clinical decision-making among intensive care nurses. Methods: The study was conducted with 152 nurses working in the intensive care clinics of a training and research hospital. Data were collected using the Descriptive Information Form, the Mindfulness in Nursing Care Scale (MNCS), the Intensive and Critical Care Nursing Competence Scale (ICCN-CS-1TR) and the Nursing Decision Making Instrument (NDMI). Data analysis was performed using descriptive frequency tables, Mann–Whitney U test, Kruskal–Wallis test, Spearman correlation analysis and binary logistic regression analysis. Results: A positive, moderate and statistically significant correlation was found between mindfulness and both the total and subdimension scores of nursing competence (respectively r: 0,563; p <0.01, r: 0,500; p <0.01, r: 0,564; p <0.01, r: 0,537; p <0.01). Positive and significant associations were found between mindfulness and knowledge-, skill-, attitude-, and value-based nursing competencies. Furthermore, the logistic regression analysis revealed that for each one-unit increase in the total mindfulness score, the likelihood of achieving an excellent level of intensive care nursing competence increased by 12.4% (OR = 1.124). No significant relationship was found between nursing competence and clinical decision-making in the study. Age, educational level, marital status and economic status were found to affect mindfulness and decision-making scores (respectively χ2=7,706; p=0,021, χ2=6,197; p=0,045, Z=-2,656 ; p=0,008, χ2=12,668; p=0,002). Conclusion: Age, marital status, educational level and economic factors were identified as variables affecting mindfulness and the decision-making process. Significant associations were identified between age, marital status, educational level, economic status, mindfulness, and clinical decision-making scores. In addition, mindfulness was positively associated with intensive care nursing competence, whereas no significant association was found between nursing competence and clinical decision-making.

YOĞUN BAKIM HEMŞİRELERİNDE BİLİNÇLİ FARKINDALIK, KLİNİK KARAR VERME VE HEMŞİRELİK BAKIM YETKİNLİĞİ ARASINDAKİ İLİŞKİLERİN DEĞERLENDİRİLMESİ

Öz

Amaç: Bu çalışma, yoğun bakım hemşirelerinde bilinçli farkındalık, klinik karar verme ve hemşirelik bakım yetkinliği arasındaki ilişkileri değerlendirmek amacıyla yürütülen tanımlayıcı kesitsel bir araştırmadır. Yöntem: Araştırma, bir eğitim ve araştırma hastanesinin yoğun bakım kliniklerinde çalışan 152 hemşire ile gerçekleştirilmiştir. Veriler, Tanımlayıcı Bilgi Formu, Hemşirelik Bakımında Bilinçli Farkındalık Ölçeği (Mindfulness in Nursing Care Scale-MNCS), Yoğun ve Kritik Bakım Hemşireliği Yeterlilik Ölçeği (Intensive and Critical Care Nursing Competence Scale-ICCN-CS-1TR) ve Hemşirelikte Karar Verme Aracı (Nursing Decision Making Instrument-NDMI) kullanılarak toplanmıştır. Verilerin analizinde tanımlayıcı frekans tabloları, Mann–Whitney U testi, Kruskal–Wallis testi, Spearman korelasyon analizi ve ikili lojistik regresyon analizi kullanılmıştır. Bulgular: Bilinçli farkındalık ile hemşirelik yeterliliğinin toplam puanı ve alt boyut puanları arasında pozitif yönlü, orta düzeyde ve istatistiksel olarak anlamlı bir ilişki bulunmuştur (sırasıyla r: 0,563; p <0,01, r: 0,500; p <0,01, r: 0,564; p <0,01, r: 0,537; p <0,01). Bilinçli farkındalık ile bilgi, beceri, tutum ve değer temelli hemşirelik bakım yetkinliği arasında pozitif yönlü ve anlamlı ilişkiler bulunmuştur. Ayrıca, lojistik regresyon analizi sonucunda, toplam bilinçli farkındalık puanındaki her bir birimlik artışın, yoğun bakım hemşireliği yeterliliğinde mükemmel düzeye ulaşma olasılığını %12,4 artırdığı saptanmıştır (OR = 1,124). Çalışmada hemşirelik yeterliliği ile klinik karar verme arasında anlamlı bir ilişki bulunmamıştır. Yaş, eğitim düzeyi, medeni durum ve ekonomik durumun bilinçli farkındalık ve karar verme puanlarını etkilediği belirlenmiştir (sırasıyla χ²=7,706; p=0,021, χ²=6,197; p=0,045, Z=-2,656; p=0,008, χ²=12,668; p=0,002). Sonuç: Yaş, medeni durum, eğitim düzeyi ve ekonomik durum ile bilinçli farkındalık ve klinik karar verme puanları arasında anlamlı ilişkiler bulunmuştur. Ayrıca, bilinçli farkındalık ile yoğun bakım hemşireliği bakım yetkinliği arasında pozitif yönlü anlamlı bir ilişki saptanırken, hemşirelik bakım yetkinliği ile klinik karar verme arasında anlamlı bir ilişki bulunmamıştır.
