HASTANE İÇİ KARDİYAK ARRESTTE OTOMATİK EKSTERNAL DEFİBRİLATÖR KULLANIMININ RESÜSİTASYON BAŞARISINA ETKİSİ

Öz

Amaç Hastanede yatan hastalarda kardiyak arrestin (KA) önlenmesi için güncel bilimsel kanıtlara dayalı resüsitasyon kılavuzlarının varlığına rağmen, hastaneden taburcu olma oranı hala düşüktür (%15-39). Her ne kadar sağlık kuruluşlarında kardiyak arrest müdahale ekipleri mevcut olsa da, potansiyel olarak ilk müdahale ekibinde yer alabilecek ve sistemi aktif hale getirebilecek tıbbi olmayan personel için de eğitim önem taşımaktadır. Çalışmamızda kardiyak arreste müdahale durumunda kalabilecek hastane personelinin resüsitasyon uygulama becerisinin otomatik eksternal defibrilatör (OED) kullanımı ile ne düzeyde değişeceğini saptamayı amaçladık. Yöntem Çalışmaya daha önce hiç resüsitasyon eğitimi almamış sağlık personeli olan 44 kişi dahil edildi. Bütün katılımcılara teorik ve pratik uygulamaya dayalı 3 saatlik Temel yaşam desteği (TYD) eğitimi European Resuscitation Council (ERC) TYD eğitimcisi olan eğitmen tarafından verildi. Eğitimde 1 saat teorik ders ve 2 saat manken üzerinde beceri uygulaması yapıldı. Katılımcılar manuel defibrilatör kullanılan (Grup MD) ve OED kullanılan (Grup OED) olmak üzere iki gruba ayrıldı. Eğitimden üç gün sonra katılımcıların TYD becerileri senaryo simülasyonu ile değerlendirildi. Bulgular Ortalama yaş Grup MD 25.52±7.59, Grup OED’de 27.23± 9.31 idi. Gruplar arasında cinsiyet dağılımı ve eğitim düzeyleri açısından fark yoktu. İki grup arasında şok uygulama süresi açısından istatiksel farklılık mevcut olup şok uygulama süresi Grup OED’de 48 (35-68.5) sn, Grup MD’de 98 (74-108) sn idi. Gruplar arasında başarılı şok uygulama oranında farklılık mevcut olup OED grubunda şok uygulama başarısı MD grubundan daha yüksekti (p<0.05) Sonuç Çalışmamızda basitleştirilmiş TYD eğitim kursundan sonra uygulayıcıların temel yaşam desteği basamakları uygulama becerilerinin her iki grupta da yüksek olduğu ancak şok uygulamada OED kullanan grubun daha başarılı olduğu bulunmuştur. Sonuç olarak çalışmamız, katılımcıların KPR ve defibrilatör özellikle OED kullanımına yönelik tutumlarını ve KPR kalitesini artırdığı için, bu eğitimlerin tıbbi kurumlarda sağlık çalışanı olmayan personel için de dikkate alınması gerektiğini düşündürmektedir.

EFFECT OF AUTOMATED EXTERNAL DEFIBRILLATOR USE ON RESUSCITATION SUCCESS IN IN-HOSPITAL CARDIAC ARREST

Objective Introduction: Despite the existence of resuscitation guidelines based on current scientific evidence for the prevention of cardiac arrest (CA) during hospitalization, the rate of discharge from the hospital is still low (15-39%). Although there are cardiac arrest response teams in healthcare institutions, training is still important for non-medical personnel who may be included in the first response team. In our study, we aimed to determine how the resuscitation application skills of hospital personnel who may be involved in CA intervention will change with the use of an automated external defibrillator (AED). Method 44 non-medical personnel who had never received resuscitation training before were included in the study. All participants were given a 3-hour basic life support (BLS) training based on theoretical and practical application by a trainer who is a European Resuscitation Council (ERC) BLS trainer. During the training, 1 hour of theoretical lecture and 2 hours of skill practice on a mannequin were performed. Three days after the training, the participants' BLS skills were evaluated with a scenario simulation. Results The mean age was 25.52±7.59 in Group MD and 27.23±9.31 in Group AED. There was no difference between the groups in terms of gender distribution and education levels. There was a statistical difference between the two groups in terms of shock application time, and the shock application time was 48 (35-68.5) seconds in Group AED and 98 (74-108) seconds in Group MD. There was a difference in the rate of successful shock application between the groups, and the success of shock application in the AED group was higher than in the MD group (p<0.05). Conclusion It was found that after the simplified BLS training course, the practitioners' skills in applying basic life support steps were high in both groups, but the group using the AED was more successful in shock application in our study.In conclusion, our study suggests that since it improves the participants' attitudes towards cardiopulmonary resuscitation (CPR) and defibrillator usage, especially AED usage, and the quality of CPR, these trainings should be considered for non-healthcare personnel in medical institutions.
