THE RELATIONSHIP OF MEAN PLATELET VOLUME LEVELS WITH MEFV AND HLA-B27 GENE ANALYSIS

Öz

Objectives: The size of platelets (MPV) in circulation is suggested to be associated with the intensity of systemic inflammation. The aim of the study was to investigate MPV influences in the patients who underwent a genetic test for MEFV and HLA-B27.

Materials and Methods: The patients who have a suspicion of Familial Mediterranean Fever (FMF) or spondyloarthropathy and underwent a genetic test for MEFV mutation (n: 126) or HLA-B27 genetic test (n: 112) were included in the study. MEFV mutation, HLA-B27 gene analysis, levels of C reactive protein (CRP), erythrocyte sedimentation rate (ESR), platelet count (PLT), platelet distribution width (PDW), platocrit (PLT), mean platelet volume (MPV) and demographic data of the patients were noted. The patients were divided into three groups according to the CRP results. Group 1: CRP > 5 mg/L MEFV positive (n: 35) or HLA-B27 (n: 53) positive. Group 2: CRP <5 mg/L MEFV (n: 53) or HLA-B27 (n: 36) positive Group 3: CRP <5 mg/L MEFV (n: 38) and HLA-B27 (n: 43) negative.

Results: MPV and PDW was significantly lower in group 1 as compared to group 2 and group 3 (p< 0,05). ESR was significantly higher in group 1 as compared to both group 2 and group 3 (p< 0,05). MPV, PDW and ESR values were comparable between group 2 and group 3. PLT and PCT groups did not show a significant difference between the groups.

Conclusion: Decreased MPV values were thought to be due to the consumption of big platelets in the inflammation side. Decreased MPV values in the patients with higher values of CRP in our study seem to support this hypothesis.

MEFV VE HLA-B27 GEN ANALİZİNİN MEAN PLATELET VOLÜM DÜZEYLERİ İLE İLİŞKİSİ

Öz

Amaç: Dolaşımdaki platelet boyutunun (MPV) sistemik inflamasyon şiddeti ile iliş-kili olduğu ileri sürülmektedir. Bu çalışmada amacımız, MEFV (MEditerranean FeVer) ve HLA-B27 gen analizi sonuçlarından yola çıkarak MPV değerlerinin anlamlı değişiklik gösterip göstermediğini ortaya çıkarmaktır.

Materyal ve Metot: Ailevi Akdeniz Ateşi (FMF) ve spondiloartropati (SpA) ön tanısı alan, birbirinden bağımsız iki hasta kümesi üzerinde değerlendirme yapılmıştır. Ayırıcı tanılarını yapmak ve kesin tanılarına ulaşmak amacı ile MEFV mutasyon analizi (n: 126) ve HLA-B27 gen analizi (n: 112) istemi olan hastalar çalışmaya dahil edildi. Hastaların yaş, cinsiyet, MEFV gen mutasyon analizi, HLA-B27 gen analizi, C reaktif protein (CRP), eritrosit sedimentasyon hızı (ESR ), platelet sayısı (PLT), mean platelet volüm(MPV), platokrit (PCT) ve platelet dağılım genişliği (PDW) değerleri kaydedildi. CRP değerleri dikkate alınarak iki farklı hasta kümesi kendi arasında üç gruba ayrıldı. CRP değerleri > 5 mg/L olan gen analizi pozitif olan akut atak dönemindeki hastalar (grup 1), CRP değerleri <5 mg/L olan gen analizi pozitif olan akut atak döneminde olmayan hastalar (grup 2), CRP değeri <5 mg/L olan gen analizi negatif kontrol grubu hastalar (grup 3).

Bulgular: MEFV mutasyonu ve HLA-B27 gen analizi için yaptığımız iki farklı değerlendirmede benzer sonuçlar çıkmıştır. MPV ve PDW değerleri grup 1’de, grup 2 ve grup 3’e kıyasla anlamlı olarak düşük bulundu (p< 0,05). Eritrosit sedimentasyon hızı (ESR ) düzeyleri grup 1’de, grup 2 ve grup 3’e kıyasla anlamlı olarak yüksek bulundu (p< 0,05). Grup 2 ve grup 3 arasında hem MPV, PDW hem de ESR için istatistiksel açıdan anlamlı bir fark bulunmadı (p> 0,05). PLT ve PCT değerleri üç grup arasında anlamlı bir fark göstermedi.

Sonuç: İnflamatuar hastalıklarda akut atak durumunda büyük plateletlerin inflamasyon bölgesinde tüketilmesi hasta sonuçlarındaki düşük MPV değerlerinin sebebi olarak ileri sürülmüştür. CRP değerleri referans aralığının üzerindeyken MPV değerlerinin daha düşük bulunması bu hipotezi destekler gibi görünmektedir.
