Early Stage Localized Lyme Disease: Case Report

Abstract

Lyme disease is a multisystem infectious disease caused by spirochetes of the Borrelia burgdorferi, which are transmitted by Ixodes species of ticks. Clinical findings vary according to stage during the course of the infection. Early diagnosis of Lyme disease depend on recognized skin findings that about 80% of all cases represent skin manifestations. Principal cutaneous mark is erythema migrans. Serologic tests may be used to support the clinical diagnosis. Early diagnosis is crucial because untreated infection can result in advanced disease involving the heart, nervous system, or joints. First choice in the treatment are doxycycline and amoxicillin. A 25 year-old-women was admitted to our clinic with redness on the left leg. The patient's lesion which present tick bite history fifteen days ago, was consistent with erythema migrans and considered early localized Lyme disease. Diagnosis was supported with serologic and pathologic examinations. She's complaint improved completely after treatment with doxycycline. We presented the case to attract the attention to Lyme disease, which is most common form of tick-borne disease in people lived in rural area. Erken Evrede Lokalize Lyme Hastalığı: Olgu Sunumu

Öz

Lyme hastalığı, Ixodes türü keneler tarafından taşınan bir spiroket olan Borrelia burgdorferi'nin neden olduğu multisistemik enfeksiyöz bir hastalıktır. Klinik bulgular enfeksiyonun seyri boyunca evrelere göre değişmektedir. Lyme hastalığının erken tanısı deri bulgularının tanınmasına bağlıdır ki tüm olguların yaklaşık %80'i deri bulguları gösterir. Başlıca kutanöz işareti de eritema migrans'tır. Serolojik testler klinik tanının desteklenmesi için kullanılabilir. Erken tanı önemlidir, çünkü tedavi edilmemiş enfeksiyon kalp, sinir sistemi veya eklemleri tutarak ilerleyici hastalıkla sonuçlanabilir. Tedavide ilk seçenek, doksisiklin ve amoksisilin'dir. Yirmibeş yaşında bir kadın hasta kliniğimize sol bacakta kızarıklık şikayeti ile başvurdu. Onbeş gün öncesinde kene ısırması hikayesi olan hastanın lezyonu eritema migrans ile uyumlu bulundu ve erken lokalize Lyme hastalığı düşünüldü. Serolojik ve patolojik tetkikler ile tanı desteklendi. Doksisiklin tedavisi sonrası şikayetleri tamamen düzeldi. Olgumuzu, özellikle kırsal alanlarda yaşayanlarda, kene kökenli hastalıklardan en sık görülen Lyme hastalığına dikkatleri çekmek için sunmaktayız.
