Rektal Prolapsus için Lokal Anestezi ve Sedasyon ile Perineal Rektosigmoidektomi

Öz

Bu çalışmada yaşı ve ek hastalıkları nedeniyle genel anestezi alması riskli olan ve ileri derecede lumbal osteoartrit ve deformite nedeniyle spinal veya epidural anestezi uygulanamayan bir hastaya lokal anestezi ve sedasyon desteğiyle yapılan perineal rektosigmoidektomi (Althemeier) ameliyatı sunulmuştur. Yetmiş altı yaşında kadın hastada lokal anestezi ve sedasyon ile toplam 13 cm barsak dışarıya çekildi ve rezeksiyon yapıldı. Anterior levatoroplasti eklendi. Lidokain lokal infiltrasyonu ve sedasyon amacıyla ketamin, fentanil ve propofol destekleri ile ameliyat tamamlandı. Ameliyat 120 dakika sürdü ve kanama miktarı 50 ml kadardı. Postoperatif birinci günde oral sulu gıda başlandı. Herhangi bir komplikasyon gelişmeyen hasta postoperatif beşinci günde taburcu edildi. Takiplerinde kabızlık şikayetinin devam etmesine rağmen hastanın inkontinansı düzeldi. Postoperatif 24. ayda nüksü yoktu. Rektal prolapsusta perineal rektosigmoidektominin gerektiğinde lokal anestezi altında ve sedasyon desteği ile yapılabilmesi işleme önemli bir avantaj kazandırmaktadır. Perineal Rectosigmoidectomy for Rectal Prolapse Under Local Anesthesia and Sedation

Abstract

We presented a perineal rectosigmoidectomy (Althemeier) that was operated under local anesthesia and sedation. The patient had high risks for general anesthesia due to advanced age and co-morbid diseases. Spinal or epidural anesthesias were not suitable due to lumbal osteoarthritis and deformity. Seventy six years old woman was operated for resection of 13 cm large bowel. Anastomosis was done after pull through of the bowel and levatoroplasty was added. Operation completed by infiltration of lidocaine for local anesthesia, in addition to ketamine, fentanyl and propofol for sedation. Procedure lasted for 120 minutes and blood loss was only 50 ml. Patient started oral liquids on postoperative day one. The patient discharged on postoperative fifth day without any complication. If necessary, perineal rectosigmoidectomy for rectal prolapse can be done under local anesthesia with support of sedation. This is an important advantage of the mentioned procedure.
