Medical Home Model in Children’s Health Care

Öz

The “medical home model” has been introduced in high income countries as a health care model that provides all preventive and curative care for children with special needs under one roof. This article informs about the “medical home model” and reviews its efficacy. Key words: Health Care Delivery; Medical Home; Children.

Öz

Bütüncül İzlem Modeli (Medical Home Model) çocukların sağlık ve gelişimsel durumlarının izlenmesi, desteklenmesi ve uygun tedavilerin sağlanması için gereken tüm koruyucu ve tedavi edici hizmetlerin aynı ekip tarafından, aynı mekanda, aynı “çatı altında” verilmesini sağlayan sağlık hizmeti modelidir. Bu yazıda Bütüncül İzlem Modeli' nin özellikleri ve etkinliği gözden geçirilmiştir. Anahtar kelimeler: Sağlık Hizmeti; Bütüncül İzlem; Çocuk.
