How does the habit of teeth clenching affect the symptoms and signs of patients with temporomandibular disorders?

Öz

Objectives: We aimed to compare the symptoms and signs of patients with the diagnosis of temporomandibular disorders TMD with and without history of diurnal teeth clenching.Patients and Methods: The study was designed prospectively and thirty four patients with the diagnosis of bilateral temporomandibular joint pain and/or dysfunction were included in the study. “Patient Ouestionnaire Form” and “Patient Assessment Form” generated by the Diagnosis and Treatment Unit of Temporomandibular Disorders, İstanbul Medical Faculty, İstanbul University, were used in the eval- uation of the patients.Results: The mean ages of patients with group I, n=17 and without group II, n=17 teeth clenching habit were 28.6±10.6 and 31.5±13.8 years, respectively. The ratios of female patients in group I and group II were 76.5% and 47.1%, respectively. The age and gender showed no significant difference between two groups p>0.05 . Stress factor was significantly higher in group I and was found to aggravate pain and other complaints, according to the questionnaire p<0.05 . Pain associated with jaw move- ments of ali directions and pain radiating to region of ear were found to be significantly higher in group I p<0.05 . Total palpation index, based on physical examination of bilateral extra and intraoral masticatory muscles, neck muscles and capsule of temporo­ mandibular joints, was significantly higher in group I p<0.05 .Conclusion: Our findings indicate that habit of teeth clenching in patients with TMD affects biomechanically both muscles and joints of masticatory system being clinically presented as muscle/joint pain, and that stress factor aggravates those symptoms. This result may be useful in the treatment of symptoms and signs of patients with TMD.

Diş sıkma alışkanlığı temporomandibular rahatsızlığı olan hastalarda semptom ve bulguları ne yönde etkiliyor?

Öz

Amaç: Bu çalışmada, temporomandibular rahatsızlığı TMR olanhastalarda, öyküsünde gündüz diş sıkma alışkanlığı olan veolmayan hasta gruplarının semptom ve bulguları karşılaştırıldı.Hastalar ve Yöntemler: Çalışma prospektif olarak planlandı ve çalışmaya iki taraflı temporomandibular eklemde ağrı ve/veya disfonksiyon tanısı olan 34 hasta alındı. Hastaların değerlendirilmesinde İstanbul Üniversitesi İstanbul Tıp Fakültesi Çene Hastalıkları Tanı ve Tedavi Ünitesinin hazırlamış olduğu “Hasta Sorgulama Formu” ve “Hasta Değerlendirme Formu” kullanıldı.Bulgular: Diş sıkma alışkanlığı olan grupta grup I, n=17 yaşortalaması 28.6±10.6 yıl, kadın oranı %76.5 iken diş sıkma alışkanlığı olmayan grupta grup II, n=17 yaş ortalaması 31.5±13.8yıl, kadın oranı %47.1 idi.
