BAŞ VE BOYUN BÖLGESİ YANIK SKARLARININ TEDAVİSİNDE DOKU GENİŞLETİCİ KULLANIMI

Öz

Bu çalışmada yanık skarı nedeniyle baş ve boyun bölgesinde doku genişletici kullanımıyla elde ettiğimiz sonuçlar ve deneyimlerimiz sunulmuştur.1999 ile 2005 yılları arasında baş ve boyun bölgesinde yanık skar tedavisi için 32 hastakliniğimize başvurmuştur. Hastaların 25'i erkek 7'si ise kadındı ve yaşları 12 ile 33 arasında değişmekteydi (ortalama 21). Baş boyun bölgesindeki skarların 21'i skalpte, 7 tanesi alın ve yüz bölgesinde, 4 tanesi ise boyun bölgesindeydi. 28 hastada baş boyun bölgesine rekonstrüksiyon için, 4 hastada toraks lateraline tam kat kalınlıkta deri grefti elde etmek için doku genişletici yerleştirildi. On iki hastada 2 adet doku genişletici kullanılırken, 20 hastada bir adet doku genişletici ile onarım gerçekleştirildi. Majör komplikasyon oranı %14'tü ve bunlar 2 olguda enfeksiyon, 2 olguda ise ekspozisyondu. Tüm hastalarda doku genişletici kullanımı sonrası yeterli doku kazanıldı ve kabul edilir estetik sonuç elde edildi.

THE USE OF TISSUE EXPANDER IN THE HEAD AND NECK REGION

Öz

In this report, our experience using tissue expanders for reconstruction of burn scar in head and neck region was presented. Thirty-two patients suffering from burn scar admitted to our clinics during the period of 1999 to 2005. Twenty five of the patients were male and seven female ranging in age between 12 to 33 (mean 21).The location of burn scars were on the scalp in 21 patients, on the forehead and face region in 7 patients and remaining 4 scars were located in the neck region. In 28 patients the expanders were implanted head and neck region while in 4 patients it was placed to lateral thorax for full thickness skin graft harvesting. Two expanders were used to reconstruct burn scar in twelve patients whereas one expander created adequate tissue for reconstruction of burn scar in 20 patients. Major complication rate was 14 %. They were implant exposure (two) and infection of expande(two). In all cases, with using tissue expansion technique adequate soft tissue was obtained to close the created soft tissue defect and satisfying aesthetic results were achived.
