Venöz Bacak Ülserinde Kanıt Temelli Uygulamalar

Öz

Venöz bacak ülseri, altı haftadan uzun süre devam eden, ayak bileğinden baldırın ortasına kadar uzanan açık bir lezyon olarak tanımlanır. Bu durum bölgesel basınç artışı ile ilişkilidir. Venöz bacak ülserleri alt ekstremitede en sık görülen ülser tipidir ve tahmini insidansı yaklaşık %1'dir. Uzun dönem tedavi ve nüks oranlarının %50-70 olduğu tahmin edilmektedir. Bu faktörler hastaların yaşam kalitesi üzerinde önemli bir etkiye sahiptir ve sağlık sistemleri üzerinde önemli bir yük oluşturmaktadır. Venöz bacak ülserlerinin tedavisinin amacı venöz basıncı azaltmak, yara granülasyonunu uyarmak ve epitelizasyonu ve yara iyileşmesini hızlandırmak, ödemi azaltmak, ağrıyı azaltmak ve hastanın yaşam kalitesini artırmak ve nüksü önlemektir. Tanı, tedavi ve takipte başarı şansını artırmak için multidisipliner bir yaklaşım önemlidir.
Bu derlemenin amacı, venöz bacak ülserlerinin önlenmesi ve yönetimi için Kanada Yara Bakım Derneği (Wounds Canada) tarafından geliştirilen kanıta dayalı yaklaşımları paylaşmaktır.

Evıdence -Based Applıcatıons In Venous Leg Ulcer

Öz

A venous leg ulcer is defined as an open lesion extending from the ankle to the mid-calf, persisting for more than six weeks. This condition is associated with increased regional pressure. Venous leg ulcers are the most common type of ulcer in the lower extremity, with an estimated incidence of approximately 1%. Long-term treatment and recurrence rates are estimated to be 50-70%. These factors have a significant impact on patients' quality of life and place a significant burden on healthcare systems. The aim of treatment of venous leg ulcers is to reduce venous pressure, stimulate wound granulation and accelerate epithelialisation and wound healing, reduce oedema, reduce pain and improve the patient's quality of life and prevent recurrence. A multidisciplinary approach is important to increase the chance of success in diagnosis, treatment and follow-up.
The aim of this review is to share evidence-based approaches developed by Wounds Canada for the prevention and management of venous leg ulcers.
