Non-surgical Periodontal Treatment of Amlodipine-Induced Excessive Gingival Growth

Öz

Drug-induced gingival enlargement is proportional to the inflammation caused by poor oral hygiene. In this case report, gingival enlargement due to the use of the antihypertensive drug amlodipine, which is a Ca channel blocker derivative, is presented. In the anamnesis taken from a 56-year-old female patient who applied to Ankara University Faculty of Dentistry Periodontology Department with complaints of swollen and bleeding gingival enlargements, pain and discomfort when chewing, it was learned that she had been using 10 mg amlodipine (Norvasc®, Pfizer, USA) daily for 6 years due to hypertension. During clinical examination, it was determined that the patient had poor oral hygiene and the presence of gingival growths. In the patient's treatment, the hypertension medication was first changed to an alternative with cardiology consultation. The patient underwent dental scaling, root planing and oral hygiene training. Growth regression was observed at the patient's 1st, 3rd and 6th month follow-up visits.Following medication change and non-surgical periodontal treatment, at the 1-year follow-up, the gingival growths had completely regressed, no bleeding was detected on probing, and plaque control was achieved. The patient gained oral hygiene habits and left satisfied. In cases of gingival enlargement due to amlodipine use, drug replacement, tooth surface cleaning, root planing, plaque control, oral hygiene motivation and regular patient follow-up play a key role in the treatment of these diseases.

Amlodipine Bağlı Aşırı Diş Eti Büyümesinin Cerrahi Olmayan Periodontal Tedavisi

Öz

İlaca bağlı diş eti büyümeleri kötü ağız hijyeninin yol açtığı enflamasyon ile orantılı olarak seyreder. Bu olgu sunumunda Ca kanal blokeri türevi olan antihipertansif ilaç amlodipin kullanımına bağlı olarak görülen diş eti büyümesi, tedavisi ve takibi yer almaktadır. Ankara Üniversitesi Diş hekimliği Fakültesi Periodontoloji Anabilim Dalı’na şiş ve kanamalı diş eti büyümeleri, çiğnemede ağrı ve rahatsızlık şikayetiyle başvuran 56 yaşındaki kadın hastadan alınan anamnezde hipertansiyon nedeniyle 6 yıldır günlük 10 mg amlodipin (Norvasc®, Pfizer, USA) kullandığı öğrenildi. Klinik muayenede hastada oral hijyenin zayıf olduğu ve gingival büyümelerin varlığı tespit edildi. Hastanın tedavisinde ilk olarak kardiyoloji konsültasyonu ile hipertansiyon ilacı alternatifiyle değiştirildi. Hastaya diş taşı temizliği, kök yüzeyi düzleştirme işlemleri yapıldı ve oral hijyen eğitimi verildi. Hastanın 1., 3. ve 6. ay kontrollerinde büyümelerde gerilemeler gözlendi. İlaç değişimi ile birlikte cerrahisiz periodontal tedavi sonrası 1. yıl kontrolünde diş etindeki büyümeler tamamen gerilemiş, sondalamada herhangi bir kanamaya rastlanmamış, plak kontrolü sağlanmıştır. Hasta oral hijyen alışkanlığı kazanmış ve memnun olarak ayrılmıştır. Amlodipin kullanımına bağlı olarak gelişen diş eti büyümesi olgularında ilacın değişimi, diş yüzey temizliği, kök yüzey düzleştirmesi, plak kontrolü, oral hijyen motivasyonu ve düzenli hasta takibi bu hastalıkların tedavisinde anahtar rol oynamaktadır.
