Transkortikal Sensöriyel Afazide İşitsel Ayırt Etme Terapisinin Kısa Dönem Etkililiği: Olgu Sunumu

Öz

Amaç: Bu çalışmanın amacı işitsel anlaması ileri seviyede hasarlanmış transkortikal sensöriyel afazili (TSA) bir olguya uygulanan işitsel ayırt etme terapisinin kısa dönemde etkililiğini araştırmaktır. Gereç ve Yöntem: Çalışmada temel olarak denek-içi deneysel araştırma modelinden yararlanılmıştır. Terapi programını bir haftada her gün 30 dakikalık uygulanan yoğunlaştırılmış seanslar oluşturmaktadır. Bu terapi programı öncesi ve sonrasında minimal karşıtlıkları bulunan uygulama yapılmış 16 yalın çift (Trained) ve uygulama yapılmamış sekiz yalın çiftin (Untrained) işitsel ayırt edilme seviyeleri sınanmıştır. Yalın çiftlerin artikülasyon odağına göre (onset ve coda) terapi öncesi ve sonrası ayırt edilebilirlikleri de araştırılmıştır. Veri setlerinin güvenirlik analizi için yalın çiftlerin %60’ında değerlendiriciler arası istatistiksel uyum değerlendirilmiştir. Bulgular: Olgunun işitsel ayırt etme terapisi öncesi ve sonrası işitsel ayırt etme becerisinin (Trained) yapılarda (p=0,048) ve artikülasyon odağının başta olduğu (onset) yalın çiftlerde (p=0,025) istatistiksel olarak değişim gösterdiği gözlenmiştir. (Untrained) yalın çiftlerde (p=0,789) ve artikülasyon odağı sonda (coda) olan yalın çift biçimlerinde (p=0,826) işitsel ayırt edilme seviyesi terapi sonrasında değişmemiştir. Değerlendiriciler arası güvenirlik sonuçları veri setlerinin %60’ında iyi uyum seviyelerini göstermektedir (K=0,838 p=0,000). Sonuç: Sonuç olarak işitsel ayırt etme terapisi kısa dönemde TSA’lı olgunun işitsel ayırt etme fonksiyonlarındaki çıktılarını minimal de olsa etkilemiştir. Uzun soluklu, sık seanslar içeren ve daha fazla katılımcının yer aldığı terapi etkililiği çalışmalarına gereksinim duyulmaktadır. Afazi literatüründe terapi uygulamalarına dair yapılacak çalışmalarda bir model olması açısından bu çalışmanın klinisyenlere yardımcı olacağı düşünülmektedir.

Short-term Efficacy Results of Auditory Discrimination Therapy in Transcortical Sensory Aphasia: Case Report

Öz

Objective: The aim of this study was to investigate the short-term effectiveness of auditory discrimination therapy applied to a patient with transcortical sensorial aphasia (TSA) whose auditory comprehension was severely damaged. Methods: Basically, within-subject experimental research model was used in the study. The therapy program consisted of 30 minutes sessions every day for a week intensively. The auditory discrimination levels of 16 trained and eight untrained minimal pairs were tested before and after the therapy program. The discernability of the minimal pairs according to the articulatory focus (onset and coda) before and after therapy was also investigated. For the reliability analysis of the data sets, inter-rater statistical agreement was assessed in 60% of the minimal pairs. Results: It was observed that the auditory discrimination skills of the case before and after the therapy showed a statistical change in trained structures (p=0.048) and in the minimal pairs of which the articulation focus were on the onset (p=0.025). The levels of auditory discrimination did not change after therapy in the untrained minimal pairs (p=0.789) and in the minimal pairs of which articulatory focus were on the coda (p=0.826). Inter-rater reliability results showed good levels of agreement in 60% of the data sets (K=0.838 p=0.000). Conclusions: In conclusion, auditory discrimination therapy made a minimal difference in the outcomes of auditory discrimination functions of the patient with TSA in the short term. There is a need for long-term therapy effectiveness studies with frequent sessions and more participants. It is thought that this study will be helpful for clinicians in terms of being a model for future studies on therapy applications in aphasia literature.
