POLİOMYELİTTE ALT EKSTREMİTE EŞİTSİZLİĞİNİN DEĞERLENDİRİLMESİNDE OBJEKTİF VE SUBJEKTİF ÖLÇÜM YÖNTEMLERİNİN KARŞILAŞTIRILMASI

Öz

Çalışmada poliomyelit sekeli olan 56 çocukta alt ekstremite eşitsizliğinin değerlendirilmesinde subjektif bir değerlendirme yöntemi ile farklı objektif ölçüm yöntemleri karşılaştırılmıştır. Yatarken spina iliaca anterior superior (SIAS)-medial malleol (MM), umbilikus-MM uzunluk ölçümleri ve ayakta SIAS’ların eşitlendiği takviye yüksekliği ile 3 farklı ölçüm yapılmıştır. Subjektif ölçüm yöntemi olarak hastanın kendini rahat hissettiği takviye yüksekliği kabul edilmiş ve bu yöntem diğerleri ile karşılaştırılmıştır. Yatarken SIAS-MM ölçüm yöntemi ve ayakta takviye ile SIAS’ların eşitlendiği ölçüm yönteminin, hastanın subjektif değerlendirme yöntemi ile aynı sonucu verdiği (p>0. 05), buna karşılık umbilikus-MM ölçüm yöntemi ile subjektif yöntem arasında fark olduğu (p<0. 05) bulunmuştur. Ekstremite eşitsizliğinin belirlenmesinde birçok yöntem olmasına rağmen, subjektif yöntemle aynı sonucu vermesi nedeniyle SIAS-MM ve SIAS’ların eşitlendiği takviye ile ölçüm yöntemlerinin klinikte pratik önem taşıdığı ancak pelvik obliklikten kaynaklanan rölatif kısalığı ayırt etmekte kullanılan umbilikus-MM uzunluk ölçümünün ayakkabı takviyesi verilirken kullanılmaması gerektiği sonucuna varılmıştır

COMPARISON OF AND SUBJECTIVE MEASUREMENTS IN ASSESSING FOR LIMB LENGTH DISCREPANCY DUE TO POLIOMYELITIS

Öz

The subjective and different types of objective measurement methods used for assessing leg length discrepancy in 56 poliomyelitis sequelled children were compared in the study. Three objective measurements namely spina iliaca anterior superior (SIAS) – medial malleol (MM), umbilikus-MM with the patient in the supine position and measuring of the sole lift required to level both SIAS in the standing position were performed for each patient. These results were compared with the lift required for the patient to perceive her/himself as level. The comparison showed that there was no significant difference in measurements of SIAS-MM, the lift required to equalize both SIAS while weight bearing and the lift required for the patient to perceive her/himself as level (p>0. 05). There was a difference between umbilicus-MM and the patients subjective perception (p<0. 05). It was concluded that while there are various methods for assessing leg length discrepancy, SIASMM and putting a lift under the sole till SIAS level off has a practical advantage since it coincides with the patients feeling of comfort and while umbilicus-MM is relevant in determining relative discrepancy due to pelvic obliquity it should not be employed while giving shoe lifts
