ERİŞKİN NÖROMUSKÜLER HASTALIKLARDA FONKSİYONEL DÜZEY İLE ANKSİYETE, DEPRESYON VE YAŞAM KALİTESİ ARASINDAKİ İLİŞKİ

Öz

Nöromusküler hastalıklarda (NMH) çok yönlü birçok problemin bir arada bulunması nedeniyle hastaların tedavi ve bakım süreçleri konusunda zorluklar yaşanmaktadır. Kronik NMH süreci hastanın bir yandan günlük yaşamını, diğer yandan emosyonel sağlığını olumsuz etkileyerek yaşam kalitelerinde kötüleşmeye neden olur. Bu çalışmanın amacı NMH olan erişkin hastaların fonksiyonel düzeyleri ile depresyon, kaygı ve yaşam kalitesi arasındaki ilişkiyi araştırmaktı. 52 erişkin NMH’lı hastanın (35, 44±16, 78) değerlendirildiği çalışmada hastaların özellikleri ve hastalık süreleri kaydedildi. Hastaların fonksiyonel bağımsızlık düzeyleri Fonksiyonel bağımsızlık ölçeği (FBÖ), depresyon düzeyi Beck Depresyon Ölçeği (BDÖ), kaygı düzeyi Durumluluk ve Sürekli Kaygı Ölçeği (SKÖ-I ve SKÖ-II) ile ve sağlıkla ilgili yaşam kalitesi (SİYK) algısı ise Nottingham Sağlık Profili (NSP) ile değerlendirildi. Çalışmanın sonucunda, FBÖ mobilite puanlarının yaşam kalitesinin sosyal izolasyon, fiziksel aktivite ve toplam NHP puanı ile ilişkili olduğu (sırasıyla; r: -, 352, r: -, 399, r: -, 322), FBÖ lokomosyon puanlarının yaşam kalitesinin uyku, sosyal izolasyon, fiziksel aktivite ve toplam NHP toplam ile ilişkili olduğu (sırasıyla; r: -, 295, r: -, 404, r: -, 546, r: -, 301) gözlendi. FBÖ toplam puanının ise yaşam kalitesinin uyku, sosyal izolasyon, fiziksel aktivite (sırasıyla; r: -, 320, r: -, 309, r: -, 412) ile ilişkili olduğu belirlendi (p<0, 05). Hastanın fonksiyonel düzeyi ile anksiyete, depresyon arasında ise ilişkiye rastlanmadı (p>0, 05). Çalışmada erişkin NMH ‘da hastaların fonksiyonel düzeyinin yaşam kalitesinin özellikle sosyal izolasyon ve fiziksel aktivite parametreleri üzerinde etkili olması hastaların toplumsal yaşama katılımının fonksiyonel düzeyden doğrudan etkilendiğini gösterdi. Ayrıca ambulatuar düzeydeki hastalarda emosyonel sağlıkla fonksiyonellik arasında bir ilişki bulunmamakla birlikte, toplumsal katılımdaki yetersizliğin, hastalığın ileri dönemlerinde emosyonel sağlığı olumsuz etkileyecek bir faktör olabileceğini düşündürdü

THE RELATIONSHIP BETWEEN FUNCTIONAL LEVEL AND ANXIETY, DEPRESSION AND QUALITY OF LIFE IN ADULT NEUROMUSCULAR DISEASES

Öz

Due to multifaceted problems associated with Neuromuscular Diseases (NMD), it has been experienced difficulties in treatment and caring process. Chronic and progressive course of NMD affects negatively both daily living activities and emotional health and may cause worsening of the quality of the life. The aim of this study was carried out the relationship between functional level and anxiety, depression and quality of life levels in patients with NMD. 52 patients were evaluated and characteristics of patients and duration of illness were recorded at the beginning of the study. Functional level was evaluated by using Functional Independence Measurement (FIM), depression level was evaluated by using Beck Depression Inventory (BDI) and anxiety level was evaluated by State and Trait Anxiety Inventory (STAI-S, STAI-T). Nottingham Health Profile (NHP) was used to evaluate for health related quality of life of patients. As a result of study, mobility parameter of the FIM was correlated with social isolation, physical activity domains and total score of NHP (respectively r: -, 352, r: -, 399, r: -, 322). It was observed that the meaningful correlation between locomotion parameter of FIM and sleep, social isolation, physical activity domains and total score of NHP (respectively r: -, 295, r: -, 404, r: -, 546, r: -, 301). Also total scores of FIM was correlated with the sleep, social isolation and physical activity domains and total score of NHP (r: -, 320, r: -, 309, r: -, 412), (p<0, 05). Functional level of the patient and anxiety and depression level did not show correlation (p>0, 05). In this study, due to correlation between functional level and quality of life, participation of the patient to the community life, affected directly from functional level of the patient, in adult NMD patients. Moreover, in the ambulatory patients, although the lack of the correlation between emotional health and functionality, impairment of the participation to the community life may be a risk factor for emotional health in the late stage of the disease
