Giant Stoma Prolapse Due to Neglected Fecalith Obstruction: A Case Report

Objective: This case report presents a rare instance of giant stoma prolapse caused by a large fecalith, stemming from neglected stoma care and lack of regular follow-up. Results: A 24-year-old female with a permanent loop colostomy presented with increasing abdominal pain and an approximately 20x15 cm irreducible prolapsed bowel loop. Preoperative CT revealed a 15 cm fecalith obstructing the sigmoid colon. Open surgical repair involved fecalith removal, bowel resection, and creation of a new end colostomy. Discussion and Conclusion: This case underscores the diagnostic importance of imaging, the necessity of surgical intervention for symptomatic or complicated hernias, and the significant challenges posed by neglected care, extensive adhesions, and compromised fascial integrity. It reiterates the critical role of comprehensive patient education and long-term follow-up in preventing and managing severe ostomy complications.
