Yoğun Bakım Hastalarında Konstipasyon ve Diyare

Öz

Defekasyon vücudun temel fonksiyonlarından olmasına rağmen konstipasyon ve diyare yoğun bakım hastalanın major gastrointestinal sistem komplikasyonu olarak karşımıza çıkar. Konstipasyon ve diyare hastalık değil, genellikle başka bir hastalığa sekonder olarak gelişen semptomlardır. Yoğun bakım hastalarının %60’ında en az bir gastrointestinal sistem problemi görülür. Bu hastaların %50 sinden fazlasında konstipasyon görülürken %50’ ye yakınında da diyare görülür. Yoğun bakımda, kafa travması yaralanma, multi sistem yetmezliği, enteral beslenme, mekanik ventilasyon, hipoperfüzyon, şok, dehidratasyon, endojen ve egzojen opoidler gibi inflamatuar mediatörlerin salgılanması, gibi durumlarda konstipasyon ile sık karşılaşılır. Enteral beslenme, enfeksiyonlar, ilaçlar, hipoalbuminemi, pseudomemranöz kolit, ümmünosupresif hastalıklar, malabsorbsiyon hastalıkları, sepsis, diyabet ve beş günden uzun süren açlık ise yoğun bakımdaki diyarenin başlıca nedenleridir. Konstipasyon ve diyare yoğun bakım hastalarının yaşam kalitesini, yoğun bakımda kalış süresini ve hasta maliyetini oldukça kötü etkilemesine rağmen kritik hastaların takibinde çoğu zaman barsak motilitesine çok fazla önem verilmez. Hemşireler yoğun bakımda boşaltım aktivitesini değerlendirirken bireye uygulanan yardımcı araç gereç varlığını, dışkılama sıklığını, bağırsak seslerini vb. tanılamalıdır. Bu derlemede konstipasyon ve diyare semptomlarının yoğun bakımdaki nedenleri, tedavisi ve yönetimi ele alındı.

Constipation and Diarrhea in Intensive Care Patients

Although defecation is one of the basic body functions, constipation and diarrhea occur as the major gastrointestinal system complications in intensive care patients. Constipation and diarrhea are not diseases themselves; they usually are symptoms that develop secondarily to another disease. At least one gastrointestinal system problem is observed in 60% of intensive care patients. Constipation is seen in more than 50% of these patients, while diarrhea is seen in approximately 50% of them. Constipation in an intensive care unit is frequently observed in cases such as head trauma, injury, multisystem failure, enteral nutrition, mechanical ventilation, hypoperfusion, shock, dehydration, and secretion of inflammatory mediators such as endogenous and exogenous opioids. The main causes of diarrhea in intensive care units are enteral nutrition, infections, medications, hypoalbuminemia, pseudomembranous colitis, immunosuppressive diseases, malabsorption diseases, sepsis, diabetes, and hunger for more than 5 days. Constipation and diarrhea have highly adverse effects on the quality of life of intensive care patients, duration of patient stay in the intensive care unit, and patient costs. However, usually intestinal motility is not given much importance during the follow-up of critically ill patients. The nurses should monitor the existence of assistive equipment, frequency of defecation, intestinal sounds, and so forth, while assessing excretion activity in intensive care patients. This compilation analyzed the causes, treatment, and management of constipation and diarrhea symptoms in intensive care patients.
