Kalp Yetersizliği Hastalarında Sosyodemografik ve Hastalıkla İlgili Özelliklere Göre Yaşam Kalitesinin İncelenmesi

Öz

Amaç: Kalp yetersizliği yaşam kalitesinin etkileyen önemli kardiyak bir sendromdur. Bu çalışma kalp yetersizliği olan hastalarda yaşam kalitesini etkileyen sosyodemografik ve hastalıkla ilgili özellikleri incelemek amacıyla yapıldı. Gereç ve Yöntemler: Çalışmanın evrenini 2009-2012  tarihleri arasında bir üniversite hastanesinin kardiyoloji servislerinde yatan, en az altı ay önce kalp  yetersizliği tanısı konan, 18 yaş ve üzeri olgular, örneklemini ise çalışmaya katılmayı kabul eden, çalışma kriterlerine uygun ve New York Kalp Birliğinin Fonksiyonel Sınıflandırmasına (NYHA) göre evre II, III ve IV’olan 120 olgu oluşturdu. Veriler yapılandırılmamış sorulardan oluşan anket formu, Minnesota Kalp Yetersizliği ile Birlikte Yaşam Anketi (MLHFQ) ile toplandı. Değerlendirme SSPS 15.0 programında sıklık, ortalama, Mann-Whitney U ve Kruskal Wallis Testleri ile değerlendirildi. Anlamlılık düzeyi p>0,05 olarak kabul edildi. Bulgular: Yaş ortalaması 64,29±12,50 olan bireylerin hastalık süresi 9,06±8,19 yıldır. Cinsiyet, çalışma durumu, acile başvuru sıklığı, egzersiz yapma ve NYHA fonksiyonel sınıflandırmasına göre toplam MLHFQ ve alt boyut puanları arasında anlamlı istatistiksel fark görüldü. Hastalık süresi fazla olanların emosyonel boyutta, hastaneye yatarak tedavi alan ve hastaneye yatma sayısı fazla olanların toplam MLHFQ ve emosyonel alt boyut puanları arasında anlamlı fark saptandı. Sonuç: Çalışmada elde edilen bulguların sonuçlarına göre; kadınların, hastaneye yatma sayısı fazla olanların, egzersiz yapmayanların ve ileri boyutta kalp yetersizliği olanların yaşam kalitelerinin daha kötü olduğu görüldü.

An investigate the socio-demographic and diseaserelated characteristics of heart failure patients that influence the quality of life.

Öz

Objective: Heart failure is an important cardiac syndrome that has an impact on the quality of life. This study was performed in order to investigate the socio-demographic and diseaserelated characteristics of heart failure patients that influence the quality of life. Material and Methods: The study universe was composed of cases at or above the age of 18, who were hospitalized in the cardiology service of a university hospital between the years 2009-2012 and the study sample was composed of 120 cases in Stage II, III and IV according to the Functional Classification of New York Heart Association (NYHA), who met the study criteria and consented to take part in the study. The data were collected using the questionnaire form titled Minnesota Living with Heart failure Questionnaire (MLHFQ). The data were assessed using the SSPS 15.0 software via frequency, mean, Mann-Whitney U and Kruskal Wallis Tests. The level of significance was considered to be p>0.05. Results: The disease duration of people with an average age of 64.29±12.50 was 9.06±8.19. Statistically significant differences were seen with respect to the sex, employment status, and emergency presentation frequency, exercise NYHA functional classification according to MLHFQ and sub-dimension scores. Significant differences were identified in the emotional dimension scores of those who had long disease durations and in the total MLHFQ and emotional sub-dimension scores of those who received inpatient treatment and had a high frequency of hospitalization. Conclusion: According to the results of the findings obtained in the study, it was seen that females, those who were frequently hospitalized, did not exercise and had advanced stage heart failure had poorer quality of life.
