Sırt ağrısı nedeniyle göğüs hastalıkları kliniğine başvuran hastaların değerlendirilmesi

Abstract

Amaç: Sırt ağrısı; kas-iskelet sistemi, nörolojik ya da visseral kökenli olabilen çok yönlü bir semptomdur. Genellikle kas-iskelet sistemi kaynaklı kabul edilse de, bazı durumlarda altta yatan ciddi torasik patolojilerin de habercisi olabilir. Bu çalışmanın amacı, sırt ağrısı şikayetiyle göğüs hastalıkları polikliniğine başvuran hastaların klinik, demografik ve radyolojik özelliklerini değerlendirmek ve torasik patolojilerle olan olası ilişkisini araştırmaktır. Gereç ve Yöntem: Bu retrospektif gözlemsel çalışmada, 1 Ocak 2020 ile 31 Aralık 2024 tarihleri arasında göğüs hastalıkları polikliniğine sırt ağrısı nedeniyle başvuran 493 hastanın kayıtları incelenmiştir. Hastaların demografik verileri, klinik bulguları ve toraks bilgisayarlı tomografi (BT) sonuçları değerlendirilmiş ve istatistiksel olarak analiz edilmiştir. Bulgular: Hastaların ortalama yaşı 51,8±15,3 yıl olup, %69,3’ü kadındı. En sık konulan tanı %80,3 ile miyalji idi. Hastaların %8,5’inde göğüs ile ilişkili radyolojik patoloji saptandı; en sık rastlanan bulgu plevral retraksiyon/kalınlaşma idi (%29). Torasik patoloji saptanan hastalarda öksürük (%78,6’ya karşı %29,9; p<0,001), balgam (%47,6’ya karşı %8,2; p<0,001) ve nefes darlığı (%47,6’ya karşı %27,9; p=0,008) gibi semptomlar anlamlı olarak daha yaygındı. Dikkat çeken şekilde, torasik patolojisi olan tüm hastalarda ağrı ilk 6 hafta içinde başlamıştı (p<0,001). Akut akciğer hastalıkları (pnömoni, bronşit, pulmoner emboli vb.) bu grupta anlamlı olarak daha fazla görülürken (p<0,001), patoloji saptanmayan hastalarda ise kas-iskelet sistemi kökenli tanılar baskındı. Sonuç: Sırt ağrısının en sık nedeni kas-iskelet sistemi hastalıkları olsa da, özellikle yakın zamanda başlayan ve solunumsal semptomlarla birlikte seyreden olgularda, altta yatan torasik patolojilere yönelik dikkatli bir değerlendirme gereklidir. Bu hasta grubunda erken görüntüleme ve ayrıntılı klinik inceleme, tanı gecikmesini önleyerek hasta yönetimini olumlu yönde etkileyebilir.

Evaluation of patients applying to the chest diseases outpatient clinic due to back pain

Abstract

Aims: Back pain is a complex symptom with diverse etiologies, including musculoskeletal, neurological, and visceral origins. Although commonly attributed to musculoskeletal causes, back pain may occasionally reflect serious underlying pulmonary pathology. This study aimed to evaluate the clinical, demographic, and radiological characteristics of patients presenting with back pain to a chest diseases outpatient clinic, and to investigate the potential relationship between back pain and thoracic pathologies. Methods: In this retrospective observational study, the records of 493 patients presenting back pain to the chest diseases outpatient clinic between January 1.2020, and December 31.2024, were reviewed. Demographic data, clinical features, and thoracic computed tomography findings were collected and statistically analyzed. Results: Thoracic pathology was detected on CT in 8.5% of patients. Those with thoracic pathology were more likely to present with acute back pain (<6 weeks) and respiratory symptoms such as cough (78.6%), sputum (47.6%), and dyspnea (47.6%) (p<0.01 for all). Pleural findings were the most common CT abnormalities. Patients without thoracic pathology were primarily diagnosed with musculoskeletal disorders. Early-onset back pain accompanied by respiratory symptoms showed a strong association with underlying pulmonary disease. Conclusion: Although musculoskeletal disorders are the leading cause of back pain, clinicians should maintain a high index of suspicion for underlying thoracic pathologies in patients presenting to chest clinics-particularly those with recent-onset pain and accompanying respiratory symptoms. Early imaging and detailed clinical assessment may help avoid missed diagnoses and improve patient outcomes.
