Educational quality of YouTube videos on lateral neck dissection: a multidimensional assessment using validated scoring systems and a novel anatomical demonstration score

Abstract

Aims: This study aims to evaluate the educational quality, surgical detail, and anatomical clarity of YouTube videos demonstrating lateral neck dissection (LND). The study utilized validated scoring systems (JAMA, GQS, DISCERN, OSATS). Additionally, an Anatomy Demonstration Score (ADS) designed specifically for this study was employed. Methods: A total of 100 English-language YouTube videos related to LND were included. The videos were independently rated by two expert surgeons using a five-point scoring system. Data were analyzed using reliability analyses, correlation analysis, and comparison tests, specifically the Mann-Whitney U test, with p<0.05 considered statistically significant. Results: Inter-rater reliability (ICC) across all scoring systems revealed that single measurement ranged from poor to moderate (0.364-0.593), while average measurement showed moderate to good agreement (0.534-0.744). Significant and strong correlations were identified across all scoring systems (p<0.05). Narrated videos performed significantly better than non-narrated videos in all scores (p<0.05). A comparison of quality and educational scores between academic and personal videos revealed that academic videos scored significantly higher across all scoring systems evaluated (p<0.05). Conclusion: The educational value of LND-related videos varied. In this study, the newly developed ADS showed a significant correlation with other scores. Academically sourced and the presence of narration had significantly higher educational and quality scores in the evaluation systems compared to others. It can be used as a complementary resource for surgical training, but should not replace standardized training processes for comprehensive procedures such as LND.
