Kardiyak Kataterizasyon Sonrası Gelişen Femoral Psödoanevrizmaların Ultrasonografik Kompresyon Yöntemiyle Kapatma İşlemi Sonrasındaki Kısa ve Orta Dönem Sonuçları

Abstract

Amaç: Femoral arter yoluyla yapılan işlemlerden sonra psödoanevrizma gelişim riski sıktır. USG aracılı kapatma işlemi cerrahiye alternatif tedavi yaklaşımı olarak öne sürülmektedir. Bu calışma da psödoanevrizmalı hastalarda USG aracılı kapatma işleminin etkinliğini ve güvenilirliğini test etmeyi amaçladık.
Yöntem: Femoral arter yoluyla yapılan girişimlerden sonra psödoanevrizma gelişmiş olan 100 hasta retrospektif olarak çalışmaya dahil edildi. Tüm hastaların USG kayıtları, psödoanevrizmanın çapı, lobül sayısı ve kese boyun uzunluğu ile eni açısından incelendi. USG probu ile psodoanevrizma kesesi boynuna kompresyon uygulandı ve kese boynu tromboze olana kadar bası uygulandı. İlk bası ile kapanmayan hastalara ikinci ve üçüncü kez bası uygulandı.
Bulgular: Hastaların ortalama yaşları 61.9±12.6 idi ve hastaların %52`si erkekti. Çalışma grubunun %69`unu akut koroner sendrom, %25`ini stabil anjina pektoris, ve geri kalan %6 lık kısmını koroner olmayan hastalar oluşturmaktaydı. Hastaların %56`sına koroner anjiografi, %39`una perkütan koroner girişim ve %5`ine perkütan mitral balon valvuloplasti işlemi uygulanmıştı. 100 hastanın ilk yapılan USG kapatma işlemlerinde (ortanca 38 dk`lık kompresyon süresi) %80`ninde başarılı kapanma izlendi. Başarılı kapanma olmayan 20 hastaya yapılan ikinci USG kompresyonu (ortanca 40 dk`lık kompresyon süresi) sonucunda 12 hastada tam kapanma sağlanırken, 8 hastada tam kapanma sağlanamadı. Bu 8 hastaya üçüncü kez USG ile kompresyon (ortanca 40 dk`lık kompresyon süresi) uygulandı ve 5 hastada tam kapanma sağlandı. Sonuç olarak, hasta popülasyonunun %3`ünde (3 hasta) etkili USG kompresyonuna rağmen tam kapanma sağlanamazken, hastaların %97`sinde USG kompresyonu ile başarılı tam kapanma sağlanmıştır.
Sonuç: USG aracılı kompresyon yüksek (%97) başarı oranına sahiptir ve cerrahi tedavi yerine kullanılabilir.
