Replantation of an Avulsed Matur Permanent Upper Incisor: A Case Report

Abstract

Traumatic injuries of teeth and supporting tissues usually affect school-age children. Avulsion is one of the most serious dental injuries of permanent teeth. Following this injury, the physician should start endodontic treatment quickly, if necessary, depending on the development of the root. The purpose of endodontic treatment procedures in these cases are as follows; to remove the effective microorganisms that are responsible for the progression of periapical lesions from the root canals by using appropriate intracanal medicaments, to prevent the resorptive inflammatory response, and to restore the function of the tooth by ensuring the healing of the periapical tissue in this way. In this case report, a 12-year-old female patient admitted to our clinic approximately 45 minutes after experiencing trauma at school, which resulted in avulsion of the maxillary left central incisor. Following the standard replantation procedure, the one-year follow-up results of the canal treatment performed after root canal disinfection using antibiotic-containing solutions and antibacterial agents are presented. Based on these results, although the involved tooth was clinically asymptomatic, no pathological changes were observed in the soft tissues, there was no evidence of root resorption on radiographic examinations, and the lamina dura was observed to be healthy. However, longer-term follow-ups of similar cases and further clinical studies should be considered before recommending the use of the mentioned antibiotic solutions during irrigation.

Avülse Olan Matür Daimi Üst Kesici Dişin Replantasyonu: Bir Olgu Sunumu

Abstract

Diş ve destek dokuları içeren travmatik yaralanmalar özellikle okul çağındaki çocukları sıklıkla etkilemektedir. Dişin alveol soketten tamamen çıktığı avülsiyon yaralanması, daimi dişlerde görülen en ciddi dental yaralanmalardan birisidir. Avülsiyon yaralanması sonrası dişin ağız dışında geçen süresi ve saklanma koşulları tedavinin uzun dönem prognozunu etkileyecek en önemli faktörlerdendir. Hekimin tedavi seçimi, dişin kök ucunun açık veya kapalı olması ve periodontal ligament hücrelerinin durumuna bağlı olarak değişir. Klinik ve radyografik değerlendirmeler sonucunda kökün gelişimine bağlı olarak hızlı bir şekilde endodontik tedaviye başlanmalıdır. Avülse olan dişlerdeki endodontik tedavi prosedürlerinin asıl amacı; periapikal dokuda iyileşme sağlayarak dişe fonksiyonunu yeniden kazandırmak ve uygun kanal içi medikamanlar kullanılarak periapikal lezyonların gelişiminde ve ilerlemesinde etkili olan mikroorganizmaların kök kanallarından uzaklaştırılarak rezorptif inflamatuar yanıtın önlenmesini sağlamaktır.
