Dental Discoloration in Endodontics: an Overview

Abstract

In dentistry, aesthetic teeth are restored in harmony with the gums, lips and face, aiming to provide a beautiful smile. Tooth discoloration is an important aesthetic problem. Discolored teeth can cause significant aesthetic problems, especially in the front region, and can negatively affect the person's social and emotional behaviors. Today, the desire to have beautiful and white teeth is increasing, which in turn increases the options for aesthetic applications. These applications are restorative techniques such as mechanical abrasion, composite restorations, porcelain laminates, composite and full crowns, and whitening procedures. Vital and devital whitening procedures applied in the treatment of discolored teeth are a more conservative approach compared to porcelain crowns and composite laminate restorations. Although the main subject of the endodontics branch of science is the relief of pain and the treatment of infection in the tooth, whitening of teeth that usually show color changes due to internal discolorations has become one of the current practical applications of endodontists. The process of lightening the tooth color by using a chemical agent through the oxidation of organic pigments in the enamel and dentin tissues is called whitening. Vital and devital bleaching methods are frequently preferred today for the removal of tooth discolorations. These methods can be examined under two headings as intracoronal and extracoronal bleaching. Internal discolorations caused by necrotic pulp tissue, intrapulpal hemorrhage or root canal filling materials left in the pulp chamber can be easily, economically and conservatively removed with intracoronal bleaching techniques used as an alternative to prosthetic approaches. Some complications may be encountered during these procedures. If the right methods are selected for the bleaching process and appropriate materials are preferred, the possibility of these and similar complications can be reduced and successful treatments that can meet the patient's wishes can be carried out.

Endodonti’de Diş Renk Değişikliği: Genel Bir Bakış

Abstract

Diş hekimliğinde estetik dişlerin, dişeti, dudaklar ve yüz ile uyum içerisinde restore edilerek güzel bir gülüş sağlamayı amaçlar. Diş renkleşmeleri önemli bir estetik problemdir. Renkleşme gösteren dişler özellikle ön bölgede önemli estetik sorunlara neden olarak kişinin sosyal ve duygusal davranışlarını olumsuz yönde etkileyebilmektedir. Günümüzde, güzel ve beyaz dişlere sahip olma isteği artmakta, bu da beraberinde estetik uygulamaların seçeneklerinin çoğalmasını sağlamaktadır. Bu uygulamalar; mekanik abrazyon, kompozit restorasyonlar, porselen laminalar, kompozit ve tam kronlar gibi restoratif teknikler ve beyazlatma işlemleridir Renklemiş dişlerin tedavisinde uygulanan vital ve devital beyazlatma işlemleri, porselen kronlar ve kompozit lamina restorasyonlarla karşılaştırıldığında daha konservatif bir yaklaşımdır. Endodonti bilim dalının ana konusu ağrının dindirilmesi ve dişte meydana gelen enfeksiyonun tedavisi olmakla birlikte, genellikle iç kaynaklı renkleşmelere bağlı olarak renk değişikliği gösteren dişlerin beyazlatılması, endodontistlerin güncel pratik uygulamalarından birisi haline gelmiştir. Diş renginin kimyasal bir ajan kullanılarak, mine ve dentin dokularındaki organik pigmentlerin oksidasyonu yoluyla açılması işlemine beyazlatma adı verilir. Diş renklenmelerinin giderilmesinde vital ve devital beyazlatma yöntemleri günümüzde sıklıkla tercih edilmektedir. Bu yöntemler intrakoronal ve ekstrakoronal beyazlatma olarak iki başlık altında da incelenebilir. Nekrotik pulpa dokusu, pulpa içi kanama veya pulpa odasında bırakılmış kök kanal dolgu maddeleri nedeniyle meydana gelen iç kökenli renklenmeler, protetik yaklaşımlara alternatif olarak kullanılan intrakoronal beyazlatma teknikleri ile kolay, ekonomik ve konservatif bir şekilde giderilebilmektedir. Bu işlemler sırasında bazı komplikasyonlarla karşılaşılabilir. Eğer beyazlatma işlemi için doğru yöntemler seçilirse ve uygun materyaller tercih edilirse, bu ve buna benzer komplikasyonların oluşma olasılığı azaltılıp, hasta isteğine cevap verebilecek başarılı tedavilerin gerçekleştirilmesi mümkün olabilir.
