Hirudoterapinin Nadir Bir Komplikasyonu: Kronik Lenfoid Hiperplazi (Psödolenfoma)

Abstract

ÖZ Kronik lenfoid hiperplazi (psödolenfoma), cilt stimülasyonuna neden olan provoke edici bir antijene bağlı olarak gelişen iyi huylu bir inflamatuvar yanıttır. Bu olgu sunumunda sülük tedavisi sonrası kalıcı dermatit gelişen bir hasta sunulmuştur. Amaç, son zamanlarda yaygın şekilde kullanılan sülük tedavisinin olası yan etkilerine dikkat çekmektir. 60 yaşında kadın hasta, sol lateral kolda bir ve sağ lateral kolda iki deri lezyonu ile bize başvurdu. Hastanın tıbbi öyküsüne göre, 4 ay önce omuz ve kol ağrısı nedeniyle iki kolunun lateral bölgelerine, tıbbi denetimi ve ruhsatı olmayan ‘fonksiyonel tıp’ adı altında faaliyet gösteren kayıt dışı bir merkezde sülük uygulamış. Ancak hastanın şikayetleri azalmayınca, eksizyonel biyopsi için tarafımıza yönlendirildi. Patolojik incelemeye göre kronik lenfoid hiperplazi tanısı konuldu. Hastada 2 yıllık takip süresince nüks gözlenmedi. Sülük tedavisi sonrasında kutanöz lenfoid hiperplazinin görülebileceği akılda tutulmalı ve hastalar, son zamanlarda sağlık kurumlarında uygulaması artan sülük tedavisinin olumsuz yan etkileri konusunda bilgilendirilmelidir.

A Rare Complication Of Hirudotherapy: Chronic Lymphoid Hyperplasia (Pseudolymphoma)

Abstract

Abstract Introduction: Chronic lymphoid hyperplasia (Pseudolymphoma) is a benign inflammatory response due to a provocative antigen causing cutaneous stimulation. In this case report, a patient with persistent dermatitis after leech therapy was presented. The goal was to highlight the potential side effects of leech therapy, which has gained popularity recently. Case Presentation: A 62-year-old female patient was referred to us with one skin lesion on her left lateral arm and two on her right lateral arm. According to the medical history of the patient, she applied the leeches on the bilateral arms 4 months ago due to the pain in her shoulder and arms. However, the patient's complaints did not decrease; after that, she was referred to us for excisional biopsy. According to the pathological examination, chronic lymphoid hyperplasia was diagnosed. No recurrence was observed during the two years follow-up period. Conclusion: It should be kept in mind that cutaneous lymphoid hyperplasia can be seen after leech therapy, and patients should be informed about the negative side effects of leech therapy, which has increased its application in health institutions recently.
