DİYABET TEDAVİSİNDE KULLANILAN TAMAMLAYICI VE ALTERNATİF TEDAVİLER: LİTERATÜR DERLEMESİ

Abstract

Diyabetes Mellitus diğer kronik
hastalıklar gibi tam bir iyilik  süreci
olmayan, akut ve kronik komplikasyonlara sebep olan kronik bir hastalıktır. Bu
nedenle diyabetli bireyler tıbbi tedavilerin yanı sıra hastalık sürecinde
onları desteleyecek tamamlayıcı tedavi yöntemlerine başvurmayı sıklıkla tercih
edebilmektedirler. Bu literatür derlemesinin amacı; diyabetli hastaların
kullandıkları tamamlayıcı ve alternatif tedavi yöntemlerini belirlemeye yönelik
yapılan araştırmaları değerlendirmek amacıyla yapılmıştır. Araştırmada,
2007-2017 yılları arasında PubMed ve EBSCO veri tabanlarında “tamamlayıcı ve
alternatif tıp” ve “diyabet” anahtar kelimeleri kullanılarak tarama
yapılmıştır. Çocuk hastalarla ve ratlarda yapılan çalışmalar tarama kapsamına
dahil edilmemiştir. Detaylı literatür taraması sonucunda, 211 makaleye
ulaşılmış, derleme kapsamına ise yarı deneysel, kesitsel ve tanımlayıcı
tasarıma sahip toplam 13 araştırma dahil edilmiştir. Dahil edilen çalışmaların
genel amaçları; diyabetli hastalarının kullandığı tamamlayıcı ve alternatif
tedavi yöntemlerinin sıklığını ve yöntemlerini belirlemek olduğu saptanmıştır
Araştırmalara dahil edilen toplam hasta sayısı 7.948 olup, hastaların yaş
aralığı 18-80 arasındadır. Tamamlayıcı ve alternatif yöntemleri (TAT) kullanan
hastaların yüzdeleri ise %34 - %92 arasındadır En fazla başvurulan TAT
yöntemleri ise; bitkisel tedaviler, multi-vitamin desteği, akupunktur olarak
belirlenmiştir. Literatür derlemesi sonucunda; diyabet tanısı alan hastaların
kullandığı bitkisel ürünlerin çoğunlukla hastalık gidişatını yavaşlattığı için
kullanıldığı görülmektedir. Araştırmalardaki örneklem sayılarının büyük
olmasına rağmen, araştırmaların tamamı tanımlayıcı niteliktedir.  Bu nedenle ileride yapılacak çalışmalarda,
kullanılan TAT yöntemlerinin hastaya olan etkisini ortaya koyacak şekilde
randomize kontrollü çalışmalar yapılması önerilmektedir.
