Remisyonda akromegali hastalarında aortik stifness (sertliğin ) değerlendirilmesi

Abstract

Amaç: Akromegali hastalarında en sık ölüm nedeni
kardiyovasküler olaylara bağlı gelişmektedir. Aortun sertliğin non invaziv
yöntemlerle değerlendirilmesi aterosklerozun erken tanısında oldukça yararlıdır.
Bu nedenle aort sertliği aterosklerozun bir belirteci olarak kullanılabilir.
Çalışmamızda retrospektif olarak 
remisyondaki akromegali olgularında aortik elastisite özelliklerini değerlendirmeyi ve sağlıklı kontrol grubu ile
karşılaştırmayı amaçladık
.

Gereç ve Yöntem: Çalışmaya merkezimize başvuran remisyondaki 20
akromegali  hastası  yine herhangi bir nedenle ekokardiyografisi
yapılmış 20 sağlıklı kontrol grubu dahil edildi. Aort elastisite parametreleri
olarak; aortik "strain", “beta indeksi”, "distensibilite"
alındı. Bu parametrelerin hesaplanmasında
transtorasik olarak yapılmış
ekokardiyografi (TTE) ile elde edilen aort çapları kullanıldı.
Standart formüller kullanılarak parametreler
hesaplandı.

Bulgular: Aortik strain hasta grubunda kontrol grubuna göre
düşük saptanmasına rağmen istatistiksel farklılık göstermedi.
(13.1±7.0 vs 16.6±4.1,p=0.064). Fakat aortik stiffness beta indeksi kontrol
grubundan anlamlı yüksek ve distensibilite ise anlamlı düşük bulundu.
4.0 (1.2–12.3)  2.6 (1.4–5.8), p=0.009 0.5 (0.2–1.9) 0.8 (0.4–1.5), p=0.006). Bu
sonuçlar bize akromegali grubunda aort sertliğinin kontrol grubuna göre anlamlı
olarak arttığını gösterdi. Aortik sertliği değerlendirmede kullanılan parametreler
üzerine en etkili bağımsız değişkenler sistolik ve diastolik kan basıncı
değerleri ile transtorasik ekokardiyografi ile çıkan aortadan alınan diastol ve
sistoldeki aort çapı ölçümleri olarak çıktı. Bunların yanı sıra distensibilite
üzerine cinsiyet, total ve LDL kolesterol düzeyleri de diğer etkili bağımsız
değişkenler olarak bulundu.







Sonuç: Yukarıda belirtilen parametreler ile aortik sertliğin
ekokardiyografik olarak değerlendirilmesi remisyondaki akromegali hastalarında
istatistiksel olarak farklı bulunmuştur . Bu sonuçlardan hareketle akromegali
de artmış olan kardiovaskuler riskin tedavi sonucu sağlanan remisyonda da devam
ettiğini göstermiştir.
