Marital Adjustment and Sexual Functioning in Patients with Fibromyalgia: A Cross-Sectional Case–Control Study
Abstract
Aim: Psychiatric symptoms are more prevalent among patients with fibromyalgia and may adversely affect interpersonal relationships and sexual functioning. This study aimed to examine the associations of marital adjustment and sexual functioning with pain severity, quality of life, depression, and anxiety in patients with fibromyalgia compared to healthy controls. Materials and Methods: This cross-sectional study included 50 patients diagnosed with fibromyalgia and 50 healthy controls. Participants were assessed using the Short Form-36 Health Survey (SF-36), Visual Analog Scale (VAS), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Marital Adjustment Scale (MAS), and Arizona Sexual Experiences Scale (ASEX). Results: Patients with fibromyalgia exhibited significantly higher levels of depression, anxiety, and pain severity, along with lower quality of life. No significant differences between groups were observed in marital adjustment or sexual functioning. Marital adjustment was negatively correlated with depressive symptoms and sexual functioning difficulties. Greater sexual functioning difficulties were positively associated with depressive symptoms. Depression and anxiety were moderately correlated and associated with greater pain severity and poorer quality of life. Conclusion: Although patients with fibromyalgia experience higher pain severity, depression, anxiety, and reduced quality of life, marital adjustment and sexual functioning appear to be more strongly associated with psychological symptoms than with fibromyalgia diagnosis itself. These findings highlight the importance of assessing depressive and anxiety symptoms when addressing relational and sexual functioning in patients with fibromyalgia.
