Çoklu Bölüm İçin Adil Hemşire Çizelgeleme

Abstract

Hemşire çizelgeleme tüm dünyada hastane personelini etkileyen oldukça karmaşık bir sorundur. Bu problem, hem hastanenin ihtiyaç duyduğu personel sayısını hem de hemşirelerin iş yüklerini azaltmak için ergonomik kısıtları dikkate alacak şekilde, belirli kısıtlar altında vardiyalardaki görevlere hemşirelerin aylık olarak nasıl dağıtılacağı sorusu ile ilgilenir. Hastanelerde uygulanmakta olan yedi gün yirmi dört saatlik sürekli çalışma modeli sebebiyle hemşireler fiziksel ve zihinsel iş yükü açısından zor koşullar altında çalışmaktadır. Bununla birlikte, sağlık çalışanlarının konsantrasyonlarının ve performanslarının kaliteli sağlık hizmeti vermeleri için hayati önemi bulunmaktadır. Dikkat dağılması, yorgunluk, zaman baskısı ve stres gibi faktörlerin etkisi ile sağlık personeli tarafından yapılan hatalar telafisi mümkün olmayan sonuçlara sebep olabilmektedir. Bu bağlamada, hemşire nöbet çizelgelemede dikkate alınması gereken hususların en önemlisi yeterli sayıda hemşirenin hazır bulundurulması ve her bir hemşireye eşit miktarda gündüz, gece ve hafta sonu nöbeti yazılmasıdır. Bu nedenle çalışma kapsamında Karma Tamsayılı Doğrusal Programlama (KTDP) modeli geliştirilmiştir. Literatürde toplam nöbet saatinin eşitlenmesine yönelik çizelgeleme yapılırken bu çalışmada literatürden farklı olarak hemşirelerin hafta içi gündüz-gece ve hafta sonu gündüz-gece nöbet saatleri ayrı ayrı eşitlenmiştir. Böylece hemşirelerin çalışma saatlerine bağlı olarak oluşan iş yükleri daha doğru bir şekilde dengelenerek daha adil bir nöbet çizelgesi oluşturulmuştur. Uygulama çalışması Erzurum Bölge Eğitim ve Araştırma Hastanesi’nde acil ve yoğun bakım servislerinde yapılmış ve sonuçlar tartışılmıştır.
