Franco Basaglia ve ruh sağlığı sosyolojisi

Abstract

Bu makale, İtalyan psikiyatrist Franco Basaglia’nın ruh sağlığı alanındaki müdahalelerini sosyolojik bir çerçevede analiz etmektedir. Çalışma, “demokratik psikiyatri” yaklaşımı kapsamında geliştirilen eleştirileri sosyal kontrol, damgalanma ve etiketleme teorileriyle ilişkilendirerek ele almaktadır. Basaglia, psikiyatrik kurumları Erving Goffman’ın total kurum kavramıyla kesişen biçimde, toplumsal düzeni yeniden üreten kontrol mekanizmaları olarak değerlendirmiştir. Anti-psikiyatri akımından ayrıştığı temel nokta, psikiyatrinin tümüyle reddi yerine, kurumsal yapının dönüştürülmesi ve demokratikleşmesini hedeflemesidir. Trieste’de yürütülen reform süreci, Basaglia’nın kuramsal çerçevesinin somut uygulamasını ortaya koymaktadır. Bu süreçte psikiyatri hastanesinin nüfusu yaklaşık 1200 kişiden sıfıra indirilmiştir. Kurumsuzlaştırma politikası, hastaları pasif nesneler olarak konumlandıran tıbbi yaklaşıma karşı, onları toplumsal ilişkilerin aktif özneleri haline getirmeyi amaçlamıştır. Damgalanmanın çözülmesi ve etiketlerin askıya alınması, hastaların kamusal alana yeniden katılımını mümkün kılan temel dinamikler olarak öne çıkmıştır. Bu dönüşüm, 1978 yılında yürürlüğe giren Yasa 180 ile hukuki bir zemine kavuşmuş ve İtalya’da psikiyatri hastanelerinin kapatılmasını öngören radikal bir politika değişikliğini kurumsallaştırmıştır. Reformun sonuçları, hastaların istihdam oranlarında artış, suçla ilişkilendirilme düzeylerinde düşüş ve toplumsal katılım biçimlerinde belirgin bir iyileşme şeklinde gözlemlenmiştir. Trieste modeli, Dünya Sağlık Örgütü tarafından örnek uygulama olarak tanınmış ve uluslararası ruh sağlığı politikalarının yeniden düşünülmesinde referans noktası haline gelmiştir. Basaglia’nın mirası, ruh sağlığı sorunlarının bireysel patolojilerle sınırlı ele alınamayacağını, aksine toplumsal ilişkiler, kurumlar ve iktidar biçimleriyle birlikte değerlendirilmesi gerektiğini ortaya koymaktadır.

Franco Basaglia and the sociology of mental health

Abstract

This article analyzes the interventions of the Italian psychiatrist Franco Basaglia in the field of mental health from a sociological perspective. The study examines the critiques developed within the framework of “democratic psychiatry” by relating them to theories of social control, stigma, and labeling. Basaglia conceptualized psychiatric institutions as mechanisms of control that reproduce social order, in ways that intersect with Erving Goffman’s notion of the total institution. His position diverged from the anti-psychiatry movement in that he did not seek the wholesale rejection of psychiatry; rather, he aimed at transforming and democratizing its institutional structure. The reform process implemented in Trieste constitutes the concrete application of Basaglia’s theoretical framework. During this process, the population of the psychiatric hospital was reduced from approximately 1,200 patients to zero. The policy of deinstitutionalization challenged medical approaches that positioned patients as passive objects, instead seeking to reconstitute them as active subjects within social relations. The dismantling of stigma and the suspension of labeling emerged as central dynamics enabling patients’ re-entry into public life. This transformation was given a legal foundation with the enactment of Law 180 in 1978, which institutionalized a radical policy shift by mandating the closure of psychiatric hospitals in Italy. The outcomes of the reform included increased employment rates among former patients, a decline in their association with crime, and marked improvements in forms of social participation. The Trieste model was recognized by the World Health Organization as an exemplary practice and became a reference point for the reconfiguration of international mental health policies. Basaglia’s legacy demonstrates that mental health issues cannot be reduced to individual pathology; rather, they must be analyzed in relation to social relations, institutional arrangements, and forms of power.
