Isolated Tubal Torsion in a Pregnant Woman

Abstract

Isolated tubal torsion is a rare cause of acute abdomen in pregnancy. The definite diagnosis is a very challenging task because it has no pathognomonic clinical symptoms or findings on imaging or in laboratory studies. We report a 22-week-old pregnant presenting with right lower abdomen pain radiating to her groin with nause and vomiting, who was operated with the preoperative diagnosis of adnexal torsion, adnexal mass and acute appendicites. During laparotomy right fallopian tube was found twisted among itself and became a cystic mass. Other organs were normal. The colour of tube became normal soon after detorsion so salpingectomy procedure were not performed. We conclude that isolated tubal torsion should be kept in mind in the differential diagnosis of acute abdomen in pregnancy and salpingectomy can be avoided with correct and in time diagnosis.

Gebede Görülen İzole Tuba Torsiyonu

Abstract

İzole tuba torsiyonu gebelikteki akut karın nedenlerinin nadir olanları arasındadır. Pa­tognomonik klinik semptomların, görüntüleme bulgularının veya laboratuar tetkikleri­nin olmaması nedeniyle kesin tanıya giden süreç oldukça zorludur. Bu olgu sunumunda bulantı-kusma ve kasığa yayılan sağ alt kadran ağrısı ile başvuran, adneks torsiyonu, ad­neksiyal kitle ve akut apandisit ön tanılarıyla opere edilen 22 haftalık bir gebeyi bildiriyo­ruz. Laparotomide, sağ tuba distal kısmının torsiyone olup kistik kitle haline dönüşmüş olduğu görüldü. Diğer pelvik ve abdominal organlar ise normaldi. Detorsiyon sonrasında kısa sürede tuba normal rengine döndüğü için salpenjektomi işlemi gerçekleştirilmedi. Doğru ve zamanında tanı ve tedavi ile salpenjektomi işleminden kaçınılabilecek olan izo­le tuba torsiyonu, çok nadir de olsa, gebelikte görülebilecek olası akut karın nedenleri arasında akılda tutulması gereken patolojilerden biri olmalıdır.
