Abstract

Background: In this study, we aimed to present 15 pericardial cyst cases -diagnosed and treated via surgical resection- with their demographic charecteristics and radiological views, in the light of the literature.Material and Methods:Fifteen patients operated between March 2007 - March 2013 due to pericardial cysts were evaluated retrospectively.Results: The mean age of the 9 female and 6 male patients was 50,26 . Fifty percent of male and %33.3 of the female patients were asymptomatic. In thorax computed tomography lesion average sizes were 9,13 cm , and placement was mainly in the right hemithorax (%40). Atypical placement was observed in 4 patients while 6 of the lesion sizes were larger than 10 cm. Video-assisted thoracoscopic surgery was performed in three cases, videomediastinoscopy in one, and posterolaretal thoracotomy for the remaining 11 patients. Recurrence was not detected in their average 30,13 months (1-73 months) follow up.Conclusion:Although pericardial cysts are benign lesions, complications due to growing and perforation are reported in the literature, and surgical treatment with low morbidity and mortality is recommended for definitive diagnosis and treatment. Even though we usually prefer exploration via thoracotomy minimally invasive methods should also be preferred in terms of both patient comfort and cost

PERİKARDİYAL KİSTLER: CERRAHİ TEDAVİ ALMIŞ 15 OLGUNUN ANALİZİ

Abstract

Amaç: Bu çalışmada, cerrahi rezeksiyon ile kesin tanı ve tedavisi yapılmış olan 15 perikardial kist hastasının demografik özellikleri ve radyolojik görüntüleriyle literatür eşliğinde sunulması amaçlanmıştır. Çalışma Planı: Mart 2007- Mart 2013 yılları arasında perikardiyal kist nedeniyle opere edilen 15 hasta retrospektif olarak değerlendirilmiştir. Bulgular: Hastaların yaş ortalaması 50,26 (30-74) 9’u kadın, 6’sı erkekti. Erkeklerin %50’si, kadınların ise %33,3’ü asemptomatikti. Toraks Beyin Tomografisi (BT)’de lezyon boyutları ortalama 9,13 cm (3-20) olup, yerleşimi ağırlıklı olarak sağ hemitorakstaydı (%40). Dört hastada atipik yerleşim saptanırken, altı hastanın lezyon boyutları 10 cm. den büyüktü. Olguların üçünde video yardımlı torakoskopik cerrahi (VATS), birinde videomediastinoskopi kullanılırken kalan 11 olguda posterolateral torakotomi yapılmıştı. Ortalama 30,13 aylık (1-73 ay) takiplerinde nüks izlenmedi. Sonuç: Perikardiyal kistler her ne kadar benign lezyonlar olsalar da literatürde büyüme ve perforasyona bağlı komplikasyonlar bildirilmiş olup, kesin tanı ve tedavi için düşük morbidite ve mortalite ile cerrahi önerilmektedir. Tercihimiz genellikle torakotomi ile eksplorasyon olsa da minimal invaziv yöntemler de gerek hasta konforu gerekse maliyet açısından tercih edilmelidir.
