Çocuklara Yönelik Perioperatif Güvenli Cerrahi Kontrol Listesi Kullanımının Komplikasyon Gelişimine Etkisi

Öz

Giriş: Hastanelerde görülen istenmeyen olayların yarıdan fazlası cerrahi bakım ile ilişkilidir ve bu olayların en az yarısı önlenebilir hatalardır. Amaç: Bu araştırma, bir üniversite hastanesinin çocuk cerrahi kliniğinde perioperatif güvenli cerrahi kontrol listesi kullanımının komplikasyon gelişimine etkisini incelemek amacıyla yapılmıştır. Yöntem: Kesitsel ve tanımlayıcı türdeki araştırmanın örneklemine, bir üniversite hastanesinin çocuk cerrahi kliniğinde ameliyat olan 0-16 yaş arası hastalar alınmıştır (n=217). Veriler ‘Hasta Bilgi ve Komplikasyon Formu,’ Ameliyat Öncesi Dönem Güvenli Cerrahi Kontrol Listesi Formu,’ ‘Ameliyathane Güvenli Cerrahi Kontrol Listesi Formu’ ve ‘Ameliyat Sonrası Dönem Güvenli Cerrahi Kontrol Listesi Formu’ ile toplanmıştır. Çalışmada, 117 hastaya güvenli cerrahi kontrol listesi perioperatif dönemde (ameliyat öncesi, sırası ve sonrası) uygulanmıştır. Yüz hastaya hiçbir aşamada güvenli cerrahi kontrol listesi uygulanmamıştır. Verilerin değerlendirilmesinde sayı, yüzde, ki kare, Fisher kesin ki-kare testi ve kappa analizi kullanılmıştır. Bulgular: Hastalarda gelişen komplikasyon oranı; güvenli cerrahi kontrol listesi uygulananlarda %4.3, kontrol listesi uygulanmayanlarda %14.0 olarak bulunmuştur. Güvenli cerrahi kontrol listesi uygulanan ve uygulanmayan hastalarda gelişen komplikasyonlar arasındaki fark istatistiksel olarak anlamlıdır (p=.011). Sonuç: Güvenli cerrahi kontrol listesi kullanımı çocuklarda görülen komplikasyon oranını azaltmaktadır. Hastanelerde çocuklara yönelik güvenli cerrahi kontrol listesinin oluşturulması ve uygulanması önerilmektedir.

Effect of Using Pediatric Perioperative Safe Surgical Checklist on Complication Development

Öz

Background: More than half of the adverse events seen in hospitals are associated with surgical care and almost half of these incidents are
avoidable errors. Objectives: This study was conducted in order to analyze the effect of using a perioperative safe surgery checklist on the
rate of development of complications in a pediatric surgical clinic of a university hospital. Methods: Patients sixteen years old and younger
who were operated in a pediatric surgical clinic of a university hospital were included in the sample of the cross-sectional descriptive study
(n = 217). The data were collected via “Patient Information and Complication Form”, “Preoperative Period Safe Surgery Checklist Form”,
“Operation Room Safe Surgery Checklist Form” and “Postoperative Period Safe Surgery Checklist Form”. 117 patients were treated with
safe surgery checklist in the perioperative period (pre-operative, intraoperative and post-operative). One hundered patients were not treated
with safe surgery checklist during any period. Number, percentage, chi-square, Fisher exact chi-square tests and kappa analysis were used in
the data evaluation. Results: Complication incidence rates in patients were found to be 4.3% in those treated with safe surgical checklist,
14%. The difference between the complications that developed in patients who were treated with safe surgical checklist and patients who
were found to be statistically significant (p = .011). Conclusions: Safe surgery checklist use reduces the rate of complications in children.
The creation of safe surgery checklist is recommended for children in hospitals and implementation
